On the States page in item ‘Reference to parent application or grant’, if reference is made to Continuation or Continuation-in-part, that can only be a non-provisional US application with number format such as12/123,456 or 13/123,456.
Signing the request form: The format preferred by the RO/US for the agent's text-string or s-signature in PCT-SAFE applications is: /John Smith, Reg. No. 9999/ or /John Smith, Reg. # 9999/.
