5. Impact The goal was 3% or less re-hospitalizations.
Clients that choose to participate averaged 2% re-hospitalization. Clients who choose not to participate averaged 9% readmission rate.
Clients feel support upon discharge.
Enhance staff morale.

Increased emphasis on hand offs and crisis planning.
