What we’ve Learned and Where we’re going Morgan Ervin, RDN, CLE September 26, 2018 NWA

2 Fully Breastfeeding at 2 Months by Ethnicity
March 2015 African American -16.3% Asian – 18.7% Hispanic – 23.5% White % Here is a snapshot of our Breastfeeding Data at the PHFE WIC program This is the chart that we reviewed as we begin looking internally at our breastfeeding gaps. We noticed that consistent with the Data, African American Rates were lower than our other Sisters. As you can see, our Asian and Hispanic sisters have decreased breastfeeding rates as well. This CinnaMoms model can be used to address their breastfeeding rates as well. The Raptor system provides caseload analysis reports for California WIC Local Agencies Birthrates are usually higher at birth Asian sisters, Hispanic sisters. This model could work to address their unique needs… PHFE WIC Internal Data System

3 Fully Breastfeeding at 2 Months by Ethnicity
March 2018 African American % Asian – 22% Hispanic – 20.4% White – 20.2% The CinnaMoms Effect. Retention, families with the CinnaMoms intervention were more likely to breastfeed than those who were not in the intervention. Keep in mind this is just a snapshot but the data is consistent with LA county- Health Indicators For Women in LA county 2017. Percent of women with a recent live birth who report exclusively breastfeeding at 3 months in LA County was 31.5%. Breastfeeding initiation for Black women increased from 68% in 2011 to 76% in 2015. Health Indicators for women in LAC- 2017 3. Los Angeles County Department of Public Health, 2014 Los Angeles Mommy and Baby Survey (LAMB). Estimates based on self-reported data by a stratified random sample of 6,035 Los Angeles County women

We need justification for creating CinnaMoms and program for African Americans at the PHFE WIC program. We know that social determinants of health are quantified using categories such as income, racial or ethnic descriptors, or geographic descriptions. When we created CinnaMoms, we decided to create breastfeeding support groups that would strategically target densely populated African American communities with low breastfeeding rates. The six sites are located in: Gardena Inglewood Rodeo St. Andrews Place (South Los Angeles Slauson 117th All of which target different areas in metro Los Angeles and serve a high African American population. At PHFE WIC we serve: 200,000 participants each month ~13,000 African American families (5.9%) 3,300 African American Infants 1,500 African American prenatal women 1,100 African American breastfeeding women ADD A FOOTER

These are all barriers preventing AA women from successfully bf. August 30th event we are redefining black motherhood and it will continue to focus on this as we move forward. Starting by raising awareness in our Agency on the Health Disparities affecting our AA participants. Encouraging all sites to refer their AA participants to CinnaMoms. We are continuously evaluating CinnaMoms and its affect on our participants & the community which has led us to our new focus which I discussed previously.

13 CinnaMoms Toolkit: Breastfeeding
Next Steps CinnaMoms Toolkit: Breastfeeding Pregnant/Maternal/Infant Health Awareness Trauma Care/Transgenerational Trauma Self-Care 4 topics we will continue to talk about at our CinnaMoms circles of support. We picked these topics because they cover the layers that need to be addressed before we can get to breastfeeding. Breastfeeding: We saw the need in our community because of the low numbers in LA County Pregnant(HTN, GDM)3 pn visits/Maternal(PPD)/Infant (PTB)Health Issues: These issues need to be detected & addressed early to prevent further health issues and complications w/bf Trauma Care/Transgenerational Trauma Questions: (Trauma)We saw staggering numbers of Africans giving birth same rates as whites sisters which declined with first generation of African Americans. (TT definition) Historical troubles and negative consequences passed down and across generations and we see this everyday in our WIC offices Self-Care Questions: We forget to take care of ourselves as we are busy taking care of others. We want to encourage & empower our moms to take time for themselves on a regular basis.
