5. How does it work? Teacher provides redirection for behavior.
Upon continued behavior, student receives a Conduct Cut stamp in the agenda, which serves as parental notification of behavior. It must be signed by a parent or guardian.
Teacher logs Conduct Cut into a Google Form.
Repeated Conduct Cuts add up to bigger consequences:
2 Conduct Cuts = Parent Phone Call
3-4 Conduct Cuts = Silent Lunch, Recess Detention, or After School Detention
5 Conduct Cuts = Office Referral
PBIS Team reviews Conduct Cut data monthly to look for trends and problem solve.

**Since our school uses Google for , usernames are automatically collected when a teacher makes an entry. If this is not the case, an additional question would be needed for the name of the teacher making the report.

Teachers can access a spreadsheet of data to look for individual student trends across classes or within their own class.
Google Forms automatically generates graphs that show trends in the data. PBIS teams can analyze the trends in their monthly meetings to problem solve systemic issues.

Once you have a list of the items you would like included on your form, start creating each item.
Add new items by clicking the plus button on the right.

15. Tips about Form Creation
Keep the typing to a minimum—anywhere you can have multiple choice options is better—less time and less opportunity for rogue responses.
You can always make adjustments as time goes on (we certainly did!), but be aware the data generated may not be as complete as if you leave it the same for the duration.
You can make a copy of the form to use in different quarters, semesters, or years, depending on how the data will be analyzed or utilized.

Create a short link that is easily accessible by staff and have them create a short cut for the form on their desk top using the short link.
