PREPARE CLAIMS AND INQUIRY FORMS OBJECTIVE: This practical exercise is designed to prepare you for the performance test on Prepare Claims and Inquiry Forms. CONDITIONS: Given the following: PS Form 1000, Domestic or International Claim. 2. PS Form, 2856, Damage Report of Insured Parcel and Contents.  3. PS Form 3831, Receipt for Article(s) Damaged in Mails.  4. PS Form 6401, Money Order Inquiry.  5. Standard USPS supplies and equipment.  PS Form 3533, Application and Voucher for Refund of Postage, Fees, and Services. STANDARDS: The student will: 1. Complete PS Form 1000 on a missing or damaged Insured Mail article.  2. Complete PS Form 3831 to receipt for an article damaged in the mail.  Complete PS Form 2856 for a damaged Insured or Registered Mail article.  4. Prepare PS Form 6401 for inquiry about payment of a domestic money order. 5. Prepare PS Form 3533 for refund of postage.   6. Answer multiple choice questions. SITUATION: You are the Claims Clerk at APO AE You will have four customers who will present a claim. Accept each claim by properly preparing the necessary form(s). You will have 60 minutes to complete this PE. SPECIAL INSTRUCTIONS:    1. Today's date is 19 February 2013.  2. Your APO's phone number is DSN   If any block on the Claims & Inquiry Form does not apply, then leave it blank. 1

3 SITUATION #1 SFC Thelma L. Bloom, of 2047 CIV SPT GRP presents to you the original money order receipt shown below and requests that she be issued a replacement money order. She states that the payee did not receive the money order. Assist SFC Bloom in completing the PS Form 6401 shown on the Answer Sheet to request a replacement money order. SFC Bloom has paid the $6.20 inquiry fee that’s required to process this claim. To reflect this payment, you will draw an impression of a postage label in the appropriate place on the form. 2

CSM Greene bought SGM Rodriguez a three piece silver desk pen set on 8 Feb 13 as a gift. The set was mailed in a flimsy padded envelope, and was inadequately packed. When SGM Rodriguez received the insured package in the mail, it appeared to have been ripped. After opening the package and checking the contents, he noticed that a pen from the three piece pen set was missing. Today he presents the completed PS Form 1000 and envelope. SGM Rodriguez has also brought in a catalog picture which lists the complete set for $ He does not have the original copy of the mailing receipt. He wants to file a claim for $65.00 so he can recover the cost of the pen set. SGM Rodriguez does not want the incomplete set and turns it over to the postal clerk. 5 SITUATION #3 SGT Baker filed a registered with value parcel claim, USPS informed him to bring the item into his local post office for inspection. The entire content of the article was visibly damaged beyond repair, the mailing container was a sturdy box that had been crushed and ripped open. The contents inside were wrapped in newspaper, that did not appear to be sufficient enough to protect the contents from damage. He presents the completed PS Form 1000, container, wrapper, and packaging to you. He does not have the original copy of the mailing receipt. Today's date is 19 Feb 13, you are the Claims Clerk at the Main Post Office at APO AE Yesterday TSgt Todd Osborne of 45 Chemical Co, PSC 310, APO AE received notification that an Express Mail piece he mailed to his girlfriend was not delivered within the three-day delivery standard. Today he comes to your post office with his Express Mail Customer Receipt and wants to request a refund of postage. Assist him in filling out the PS Form 3533 shown on the Answer Sheet.
