For illustrative purposes only. Reports do not reflect actual client information or all of the products or options available for each report. Clients are registered for Core Reports according to their type of self-insured service. Designed to run concurrently with the client’s payroll cycle, a combination of the following reports can be provided via or internet: Paylist Reports* (Slides 3,4 & 5) Non-checkcutting service only Details by Employee with Detail Totals and/or Summary by Employee Off-cycle Paylast reports are available, but not shown ASO Banking Reports (Slides 6 & 7) Checkcutting service only Payment Detail (by Employee) and/or Payment Summary Off-cycle reports are available, but not shown Standard Advice to Pay Reports* (Slides 8 & 9) Claimant or Claim Status View Standard Benefit Percentage Reports* (Slides 10 & 11) * If requested, Unum can also provide this information as a Trial report, allowing the client the opportunity to ensure all information is reported and accurate before the Final report is run and benefits are paid.

Paylist Report (Non-Checkcutting only) – Detail View These reports provide clients with final advice on specific dollar amounts to pay all employees for the current reporting period. There are two views available: summary and detail. The detail view shows gross benefit amount by claimant, detailing any deductions and/or offsets and citing net and final payment. (The total tab of the detail view shows individual checks, EFT and client totals.) If requested, Unum can also provide this information as a trial report, allowing the client the opportunity to ensure all information is reported and accurate before the final report is run and benefits are paid. Same page continuation Paylist Reports (PYLAST) Because the Paylist reports are designed to run concurrently with the client’s payroll cycle, there are instances where an employee would not be included in the Paylist reports and would not get compensated until the next payroll cycle. For these instances, the Paylast reports provide the client with final advice on specific dollar amounts to pay any employees that were not included in the Paylist reports run on the last payroll cycle. For the ease of the client, the reports have the same design as the Paylist Reports and offer summary and/or detail views.

4 Paylist Report – Detail View (Totals)
The Totals tab of the Detail view shows total payment amounts, as well as total offsets and deductions, where applicable. Paylist Reports (PYLAST) Because the Paylist reports are designed to run concurrently with the client’s payroll cycle, there are instances where an employee would not be included in the Paylist reports and would not get compensated until the next payroll cycle. For these instances, the Paylast reports provide the client with final advice on specific dollar amounts to pay any employees that were not included in the Paylist reports run on the last payroll cycle. For the ease of the client, the reports have the same design as the Paylist Reports and offer summary and/or detail views.

ASO Banking Reports (Checkcutting only)– Payment Detail Shown in three segments, for illustrative purposes only. The ASO Banking Reports are used to help customers fund their Paylist check-cutting accounts. These customers are using a new ASO banking arrangement administered through JP Morgan Chase and where disability checks are issued through Bank of America. The ASO Banking Reports are encrypted Excel files that are sent directly to the customer, via , from “Unum ASO Banking Reports”. The Payment Detail report identifies all employees that are out on claim. It breaks out the employee taxes and employer FICA match for each claim. It provides detailed information on credits (i.e., stops, reversals, and repayments) and supplies individual division numbers, claim numbers, check/EFT numbers, payment dates, and check amounts. Off-cycle ASO Banking Reports (Check-cutting only) Because the ASO Banking Reports are designed to run concurrently with the client’s payroll cycle, there are instances where an employee would not be included in the reports and would not get compensated until the next payroll cycle. For these instances, the off-cycle reports provide the client with final advice on specific dollar amounts to pay any employees that were not included in the regularly-run reports, run on the last payroll cycle. For the ease of the client, the reports have the same design as the ASO Banking Reports (Payment Detail and Payment Summary views).

ASO Banking Reports (Checkcutting only)– Payment Summary The ASO Banking Reports are used to help customers fund their Paylist check-cutting accounts. These customers are using a new ASO banking arrangement administered through JP Morgan Chase and where disability checks are issued through Bank of America. The ASO Banking Reports are encrypted Excel files that are sent directly to the customer, via , from “Unum ASO Banking Reports”. The Payment Summary report removes claimant detail information in order to protect the privacy of those employees that are out on disability. It advises of amount due to Unum for claim reimbursements (‘Please fund this amount: $$$’) and is used as a reference for sending funds. It can be run by claimant or by claim status. This report is designed to run concurrently with a client’s payroll cycle. If requested, Unum can also provide this information as a trial report, allowing the client the opportunity to ensure all information is reported and accurate before the final report is run and benefits are paid.
