2 2 Available Time Report This CR&A report provides regularly updated calculations of leave entitlement (time taken and time available) for the federal, state and/or corporate leaves for which the employee is eligible.

3 3 This report shows all updates or modifications to FMLA leave data (not all fields are shown here). If there were no activity triggers on an open leave, the leave would not show up on the daily report. Examples of activity triggers include first reports of leave, changes in leave dates, changes in FMLA/State leave status, intermittent days reported and return to work confirmations. The report may also be scheduled on a weekly basis to reflect cumulative changes in the prior week. Daily Absence Report

4 4 Detail Integrated Absence Report The leave number lookup feature contains three tabs of information. The Intermittent Hours and Protections detail tabs are not illustrated here. The report can be run for a date range of your choosing and includes 27 fields of information (all fields are not represented in this example). For more detailed information on individual claimants, including a history of intermittent absences reported or STD payment detail, a user can utilize the on-line lookup and drill-down applications (with the option to print the information).

5 5 This report provides a summary of all intermittent absences. The report can be run for any period of the user’s choice. Sub-totals of all approved intermittent absences per employee are provided. Intermittent Leave Summary Report

6 6 This on-line search tool provides quick and easy access to detailed claim and/or leave information at the employee level. Employers may search for claim/leave information by simply typing an employee's name, SSN, claim number, or leave number. With several layers of drill-down, this tool lets employers quickly view a wealth of important information about an employee. Home Page Claim/Leave Search

7 7 Leave Events By Type This report provides a summary of all leave events by type (Full, Intermittent, Reduced Schedule) and status (Approved, Pending, Denied). Users can select up to 12 quarters of historical data. The report charts leave type values and allows users to trend approval and denial rates. 8 8 Leave Events By Reason This report provides a summary of all leave events categorized by leave reason (EE Health, Parent Health, Spouse Health, etc.) and status (Approved, Pending, Denied). 9 9 This report generates a list of employees whose return to work has been confirmed by Unum within the specified review period. Review period options include the previous or current week, the previous or current month, or any start and end date that the employer chooses. Return to Work Confirmation Report

10 10 Total Lost Time – Leave Repeater Report This report reveals important information about recurrent leave patterns. It tracks the number of employees with more than one leave request in a particular time period and lets employers see if "repeaters" have a disproportionate number of lost work days. This report includes approved and denied leave requests but will not include leave requests processed in error, historical leaves (leaves processed prior to the Leave Management program with Unum), or canceled leave requests (leaves requested but later canceled.) This report may be run for individual STD policies, individual report groups or it may be run for all STD policies and all report groups. Employers also have the option to view this report by leave category: Medical, Family, Other or All.
