We have made a personal website that is dedicated to your plan; it’s all there, right at your fingertips. How can you access your personal webpages? It’s as easy as 1, 2, 3. Plan members 1 Go to and click ‘Plan members’. Plan members Personal reference number: Continue Fill in your personal reference number, which you will find on your membership card. 2 3 On the next screen, fill in your password.

16 Medical network Your needs are what drives our network’s expansion, that’s why we continuously monitor it to make access to affordable quality care a reality. Constant monitoring New preference Our own direct network evolves with the needs of our clients: We add new providers when new clients sign up to Cigna, we ask them to provide names of providers they would like to see signed to the network; or If we see that our client's employees are using certain providers, we will start the screening process and, if positive, sign them up to our network. This ensures that the network is kept aligned to client needs Our network allows that plan members benefit from the convenience of being treated within the network e.g. their bills are being settled directly by Cigna, there is no need to front bills and re-claim, they enjoy discounts. On the other hand, plan members keep complete freedom of choice of hospitals, doctors and specialists. If they visit out-of-network providers, we will make the necessary arrangements to make them enjoy the same benefits. The network is constantly being monitored. When necessary, this means we also take providers out of our network (eg fraud, poor quality).

20 How to file a claim (by post)?
Step 1 Step 2 Step 3 Lisa goes to her personal webpages and logs in. She clicks ‘Claims’ and downloads the claim form. Lisa completes the claim form on her computer. Her personal details are completed automatically: her name, the name of her (former) employer and her personal reference number. Use this slide when you want to explain the PAPER claiming procedure.

21 How to file a claim (by post)?
Step 4 Step 5 Step 6 Lisa prints the claim form and signs it. She collects all relevant medical documents (hospital invoices, prescriptions, ...). She sends all documents by post to the Cigna office serving Lisa’s region. Use this slide when you want to explain the PAPER claiming procedure. Online tip You can also find your claim form on your personal webpages under ‘Useful documents’.
