9 International Emergency Dental Program Emergency dental treatment for the international traveler Access to comprehensive emergency dental benefits while working or traveling nearly anywhere in the world. Participating dentists are credentialed, speak English and meet the requirements to practice dentistry in their home countries. Emergency dental care received from an International Emergency Dental Program dentist is reimbursed in full and does not count toward the member’s annual plan maximum. Claims are paid directly to the member in U.S. currency, since the patient pays the bill at the time services are rendered. The International Emergency Dental Program is managed by DeCare Dental, LLC. DeCare Dental is an independent company offering dental management services to Anthem Blue Cross and Blue Shield. 9 CONFIDENTIAL

10 10 CONFIDENTIAL Ask a Hygienist Email Box Members can receive free personalized advice on a variety of oral health related topics from our team of licensed dental professionals. This direct interaction with members can help improve oral heath and reduce long-term costs because prevention is the key to better health. Members access the link to the new “Ask a Hygienist” email service when they log into the dental member portal. From there it’s simple – members enter their name, email address and their question(s). Once submitted, the member will receive a direct email response within 24 hours.
