3 Survey Activity The Joint Commission Point of Service Lab Survey
December 7-11 25 laboratories and services at 5 locations College of American Pathologists (CAP) Lab Survey Two-day comprehensive inspection of laboratory services at CSC, East and West Clinics CAP is accrediting body for main UWHC Clinical Laboratories The week of December 7-11 brought not only 18 inches of snow to our region, but also an unannounced Joint Commission Laboratory survey. The surveyor reviewed approximately 25 labs and lab services at 5 locations. Surveyor noted only a few isolated findings on his preliminary report involving Procedure for testing with insufficient specimen volumes. Proficiency testing Process for manual log entries Specimen retention UWHC may appeal one of the findings. Compliments far exceeded opportunities for improvement and we were praised for teamwork and dedication to service and patient care excellence. Wonderful success demonstrating our level of service even in the midst of a weather emergency. January 13 we welcomed surveyors from the College of American Pathologists or (CAP) for a Lab Survey They will be inspecting the CSC, East and West.

4 Survey Activity - continued
AABB Transfusion Services Survey Formerly known as the American Association of Blood Banks Transfusion Services including the Blood Bank, Perioperative Cell Salvage and Apheresis An AABB (formerly known as the American Association of Blood Banks) team of three surveyors completed a 1-1/2 day comprehensive inspection of our Transfusion Services including the Blood Bank, Perioperative Cell Salvage and Apheresis. AABB is an international association representing individuals and institutions involved in activities related to transfusion and cellular therapies. The inspectors were colleagues from other AABB-accredited laboratories trained to perform a peer-based evaluation. The exit report addressed areas for improvement, along with commendable practices and observations. The overwhelming theme of this exit was our excellent staff! After each inspector thanked the staff they worked with, they emphasized the impact this team has on the quality of our Transfusion services -- some of the best they've seen. They described the staff as smart, knowledgable, and engaged. The inspectors cited only two items requiring correction, along with acknowleging several commendable practices and a request to submit two best practices to their peer website.

5 Accreditation Commission on Accreditation of Rehabilitation Facilities (CARF) three year accreditation of UWHC Inpatient Rehabilitation programs

6 Updates Mid-December Storm H1N1 Response Haiti Response
~ 16 inches of snow Highest # of weather related absences 117 Rides, 58 staff members housed overnight 430 Inpatients, 52 Surgical procedures Rides are provided within Dane county, but due to the intensity of the storm and poor road conditions, at approximately 11 am on Wed., 12/9, rides were decreased to within the City of Madison and City of Middleton until approximately midnight 12/9/09. In the midst of this 430 Inpatients, 52 Surgical procedures were completed. With each event, in this case the snow emergency, there are learnings. For example we identified challenges in working with the phone systems in the Nursing Resource Center and arranging rides, and we’re coordinating now to find solutions. It may also result in some updates to the Severe Weather Policy and some enhancements to some of our processes related to documentation in severe weather emergencies As a whole it was another example of how we as an organization pull together and work though challenging situations.
