3 Adult and child residential
390 people we support in 91 homes, predominantly with an intellectual disability 24/7 duty of care – rostered staff 1 – 6 individuals in each home Funded by Government based on needs

4 Adult and child residential continued
Active support, community participation, upskilling and helping to reach potential, connecting, goal attainment Outcome Brokers are responsible for realising goals Quality of Life measures Holistic approach

5 Respite facilities Nine respite facilities providing respite for 290 people 1 for babies, 4 for children and young people and for adults with disabilities Funded by Government Relief for families and enjoyable and beneficial experience for individuals attending Includes community and home based activities

6 Out of School Programmes
8 School Holiday Programmes for 200 children and young people After School Care for 8 children Weekend Day Respite for 25 children Funded by Government and family contributions

7 Home Support (In Home Respite)
Supporting 330 people between the ages of 5-65 Autism/Aspergers and Intellectual Disability main area of support 1:1 Support provided in their own home or the community Goal based support rather than personal care or household management Support hours ranging from hours per week dependent on assessed needs.

As you grow let our supports grow with you. We want to be able to offer the RIGHT support at the RIGHT times of your lives.

9 Choices in Community Living
CiCL Choice in Community Living is an alternative to residential services. It offers more choice and control over where you live, who you live with and how you are supported. The funding is managed by you and your family/whanau, in partnership with your chosen CiCL provider Choice in Community Living is for people who either: live in a residential service and who want to leave and live independently in a home they can purchase or rent live in their parent’s or family home with support needs similar to people in residential services, and who want to live independently in a home they can purchase or rent

10 Supported Independent Living
So you want to go flatting but need a bit of help and support? Supported Living is a service that helps you to live independently by providing support in those areas of your life where help is needed  A support worker will work with you, usually at your home, but support will be provided at times and places that are agreed You and your support worker will come up with a plan of things you want to achieve that help you become more independent, maybe; Making more friends How to cook How to catch buses

11 Community Cultural Support Services
Our cultural teams: Assist families to access support from government agencies Link families with other disability support agencies Assist staff and families to promote cultural understanding Encourage people to become actively involved and included in their community Link people to their culture and family/whanau Support families in the planning and reviewing of support services

12 Cultural Support Services
Our Awhi, Pasifika and Asian cultural teams provide a range of specialist services for the people we support. The teams assist people and families/whanau to access services and ensure cultural needs are both recognised and met.

13 Pacific Cultural Group
Consists of all Pacific Peoples and their families, staff Pacific Leader Appointed – Strategic Plans Pacific Specific Leaders – Fiji, Samoa, Tonga, Cook Islands, Niue Strategic Planning Meetings Luau Nights - Samoan, Fiji, Cook Island, Niue, Tongan Annual Pacific Christmas Cultural Award - SOSCARS
