10 Recent Incident Description: Incident Tracking Number: 14-0203
Date of Incident: April 11, 2014 Description: Worker picked up building in Calgary and secured load, including one strap over the top of the unit and then proceeded to travel to Hinton to offload. Worker stopped in Drayton Valley to check load and tightened strap. Worker went past Wildwood when the strap over the roof frayed and the roof blew off the building.

12 Recent Incident Findings: Corrective Actions:
Worker failed to protect the straps from sharp edges. Worker did not have the proper equipment to install load softeners on roof. Corrective Actions: Review the obligation to Stop Work and reassess when “things don’t feel right” at next safety meeting. The manufacturer of the skids will unitize (for the next loads) the lids of these skids by using wrap and card board (card board is to act as edge protectors but drivers are to ensure they are properly placed). Ladders are available at the manufacturers’ yard to use to properly place edge protection if required. Shell RT will attend future safety meetings to discuss stop work authority and expectations with ENTREC staff.  ENTREC will present these learnings at the next Shell Road Transportation forum.

You can also update your screen saver (cell, ipad, computer) with a pic of why you work safely. Risk Tolerance – ENTREC’s monthly safety activity will review the 10 factors that effect risk tolerance for the next three month. BBO – The challenge continues! Discuss with the workers to participate activity in the safety meetings for the benefit of all workers. If you are not able to attend the monthly safety meeting in person please read the meeting minutes when you arrive back at the shop. Remember ENTREC’s number one core value is Safety!
