2. RACI Chart Instructions:
The RACI chart is a tool used for identifying who will do what allowing you as the change sponsor or manager to ensure accountability for all tasks. To complete the grid:
List your milestones, tasks, or activities in the column to the far left.
Next list either specific people or departments in the space across the top of the graph.
Finally, identify who is:
Responsible by placing an R in the corresponding box that aligns to the task. There must be one “R” for each row (no more and no less).
Accountable by placing an A in the corresponding box that aligns to the task. There can be zero to one A’s in each row but no more than one. Not every task needs a key decision or approver.
Consulted by placing a C in the corresponding box that aligns to the task. There can be as many “C’s” as necessary.
Informed by placing an I in the corresponding box that aligns to the task. There can be as many “I’s” as necessary.
