2 What is Accreditation Canada?
Independent, not for profit organization that accredits health care and social service organizations in Canada and around the world. Ongoing quality improvement through evidence based standards Rigorous external peer review

3 How Does it Improve Quality?
One of the most effective ways for organizations to regularly and consistency examine and improve the quality of their services Provides a tool for organizations to embed accreditation and quality improvement activities into their daily operations Primary Focus is including the client and family as true partners in service delivery

4 Client and Family Centered Care Approach
Guides all aspects of planning, delivering, and evaluating services Focus is always on creating nurturing, mutually beneficial partnerships among the organization team members and the clients and families they serve

5 Client and Family Centered Care
Working collaboratively with clients and their family to provide care that is: Respectful Compassionate Culturally safe and competent While being responsive to their needs, values, cultural backgrounds, beliefs and preferences.

Partnership and participation Collaboration

7 Dignity and Respect Listening to and honoring client and family perspectives and choices. Client and family knowledge, values, beliefs and cultural backgrounds are incorporated into the planning and delivery of care.

8 Information Sharing Communicating and sharing complete and unbiased information with clients and families in ways that are affirming and useful. Clients and families receive timely, complete, and accurate information in order to effectively participate in care and decision making

Encouraging and supporting clients and families to participate in care and decision making to the extent that they wish

10 Collaboration Collaborating with clients and families in policy and program development, implementation and evaluation, facility design, professional education and delivery of care.

11 Accreditation – why participate?
Helps to organize and strengthen patient safety efforts. • Helps to create a culture of patient safety. • Reviews the care and services we provide against national standards. • Colleagues review our processes and identify areas of strength and areas for improvement. • Helps improvement strategies through standardization and the use of metrics. • Accreditation is mandated by Alberta Health.

13 Standards being surveyed in 2016:
• Emergency Department Services • Critical Care • Medicine Services • Emergency Medical Services (Banff)

14 Standards • System-wide standards address governance, leadership, infection prevention and control, and medication management. • Service excellence standards that address specific types of services, conditions, and populations (i.e. Medicine, Emergency Department etc.) • Standards support ongoing quality improvement activities and focus on the essential components of safe and effective health care. • Process for standard development: Comprehensive literature review Consultation comprised of experts Stakeholder feedback

15 Required Organizational Practices (ROPs)
• Evidence based practices that organizations must have in place to enhance patient safety and minimize risk. • Have Tests for Compliance: Major – immediate effect on patient safety Minor – support a patient safety culture and quality improvement but may need time to be fully developed and evaluated.

16 High Priority Criteria
• Are indicated by an ! in the standards. • Have a strong focus on risk, safety, quality improvement and/or ethics.

17 Process - Continuous Quality Improvement
Teams from clinical areas review the services that are provided against the Accreditation Canada Standards. • The teams then develop action plans to address any identified gaps. • Accreditation Canada surveyors conduct an on-site survey. • The survey team leaves a report of their findings. • Quality improvement activities continue. • Teams review standards on a regular basis and address new requirements.

18 What can you do? • Know the work that has been done in your area to address the standards and the ROPs. • Be familiar with improvements in your area to enhance patient safety. • Answer questions honestly. • If you do not know the answer to a question refer the surveyor to someone who will. • Know how to access policies and procedures.
