2 All exhibitors must submit entry materials to their county office by the deadline established by the county. Entry form(s) and entry fees for each animal entered Registration papers if showing in registered In Hand class Completed and signed medical consent/parental authorization forms Proof of negative EIA status (Coggins Test) for all horses Proof of current rabies vaccination by a licensed veterinarian Veterinary Client Patient Relationship statement (VCPR) (OR record that VCPR statement is on file in county extension office*) Stall reservation form and payment if stalling is needed Youth under 12 years of age must pass and submit a completed and signed Level 1 Horsemanship Skills Test for each animal being shown. (all applicable skills)

All entries must have copies of the following attached to each entry: Registration papers if being shown in a registered In Hand class Completed and signed medical and emergency consent form for each exhibitor

6 Record and maintain in county office
Exhibitor needs to submit Rabies, Coggins, and VCPR -- County extension office should review paperwork and verify status for: Rabies vaccination administered by licensed veterinarian (enter date of vaccination on entry form) Negative EIA test (Coggins test) Enter date of test result on entry form. VCPR statement Verification of rabies vaccination, copies of negative EIA test results and VCPR statement should be maintained by the county office. 4-H members should keep the original documents. It is not necessary to submit copies of rabies vaccination verification, negative EIA test results and VCPR IF status is verified in county office and noted on entry forms

7 Mandatory – attach for youth under 12 years of age
County staff – there is no birthdate or age entry area on this form. To assist the show office, please write the exhibitor’s birthdate next to their name Youth Under 12 Years of Age: (born after Jan 1, 2002) Must submit a copy of the Level 1 Horsemanship Skills Evaluation Sheet with entry or submit at the show for each animal. The Evaluation Sheet must include signatures of 3 trained Horsemanship Skills examiners and indicate that the youth has passed all applicable handling/riding skills.

Select on breed from 1st column OR one type from 2nd column Select mare or gelding Select age (no weanlings may be shown)

10 Horse may be entered in only ONE Futurity class
Horse may NOT show in both a walk/trot and a walk/trot/canter class.

11 Horse may be entered in only one Performance skills class
But an animal may show in one seat for a futurity class and another for the performance skills class. For example – may show in Western Futurity and English Performance Skills class.

12 Minis may show in both In Hand Trail and Driving

14 Stall Reservation Form
Stabling is encouraged, but not mandatory Stall reservations must be made when entries are submitted. No stall reservations will be accepted after June 25. Stall fee is $60.00 per stall Tack stalls may ONLY be ordered by the counties, not by individuals. Counties may reserve one tack stall for every 4 horses (see county info sheet). Tack stalls are $60.00 Counties should designate a volunteer to handle assigning individual stalls within a county block of stalls.

15 Stall Reservation Form
Bedding may be pre-ordered on the stall reservation form, or ordered at the show. Pre-ordered bedding will be delivered to the exhibitor’s stall upon arrival. Pre-ordered shavings are $6.50 per bale. Shavings purchased at the show are $7.00 per bale.

16 Camper space will be available on a "first come, first serve” basis beginning Friday, July 25 at noon. There are plenty of camping sites (all with electric and water). Please do not arrive earlier! The cost for campers is a flat fee of $55.00 for the weekend (regardless of number of days camper is parked). No advance registration accepted. Payment must be made upon arrival.

17 County Summary Sheet This sheet summarizes the individual entries and should accompany the entry forms. This is also where the COUNTY will order tack stalls. Form was supplied as an excel file. May be filled out by hand or complete in excel. You may then the completed form to the show manageer. However, please included a hard copy with the show entries when mailed.
