2018 Homeless Assistance & Public Services Community Development Block Grant (CDBG) Public Services funding Emergency Solutions Grant (ESG) Homelessness causes Lake County Video Gaming Funds (VGR) Gambling addiction and other behavioral health Identify that CDBG funding is doled out based upon location of residence of recipients. Identify that Waukegan has its own funds, handled by Laraesa and team and that North Chicago is managed by LC.

Assist People with Disabilities (Goal #7): To provide both services to people with disabilities and accessibility modifications to the homes, facilities and workplaces of people with disabilities, including the elderly. Provide Welcoming Communities (Goal #8): To foster welcoming, inclusive and neighborly suburban communities. Grant Requirements: Match All applicants must provide funding for a portion of the proposed Total Program Cost from other sources. CDBG: at least 25% of the Total Grant amount If total grant amount is $100,000, we expect you to provide $25,000 minimum towards the program. ESG: 100% of the Grant amount Dollar for Dollar match Video Gaming: No matching requirement Monika

ESG recipients are required to participate in the ServicePoint database UNLESS you are a domestic violence service program Collecting all data elements required in the HMIS data standard Must meet ServicePoint Participation requirements, which includes: Regular participation in monthly ServicePoint meetings Timely and accurate entry of minimum data standards Participation in the annual PIT and AHAR

20 ServicePoint Referral Network
ServicePoint is a web based client management tool with referral capabilities. A referral network connecting Lake County service providers via one common electronic platform has been implemented. Lake County Video Gaming fund, ESG and CDBG grantees must participate for the duration of their grant. All other organizations are encouraged to participate

21 Application Oddities for ESG Applicants
Client Information Sheet Client location – Waukegan/North Chicago/Lake County does not have the same implications as for CDBG Low/Mod Clients: Income is not an eligibility requirement for shelter Use of funds budget Please refer to ESG budget. Quick guide available here: to help determine eligible costs: program-components-quick-reference/ One time additional funding from PY2017 will be added to the 2018 funding round. It is not expected in future rounds.

26 Lake County Specific Instructions
Lake County Specific Instructions Eligible Applicants/programs must: be either units of government, public not- for-profit entities or private not-for-profit entities; be open to all residents of the county and must target beneficiaries outside North Chicago and Waukegan to be eligible for Lake County CDBG funding; have applications for: CDBG: a minimum of $10,000 and no more than the maximum of $50,000 ESG: no minimum or maximum in PY2018 Jordan Applicants may apply only for either one CDBG Public Services funding request OR one ESG funding request per agency/entity, but not both. Applicants may apply for more than one program if they are applying for homeless prevention or rapid rehousing programs under ESG.

Programs must demonstrate that funds are supporting an expansion of behavioral health services Priority is given to programs addressing problem gambling that are DASA certified and employ specially trained counselors

Waukegan Specific Instructions Applications for Waukegan Public Service activities must be a minimum of $5,000 and target residents of Waukegan Applicants will be required to do presentations before the Stakeholder Participation Panel. 25% Match Required If total program cost is $125,000, we expect you to provide $31,250 minimum towards the program. Develop 1-3 outcomes that best capture the impact of your program Outcomes should relate to the consolidated plan goal that you are applying under No outcomes will be required. Data will be pulled from HMIS as appropriate. CDBG-Public Services and VGR ESG

33 Outcome Definition An outcome is a problem-related, attainable, and measurable statement of a program’s intended effect on its client’s knowledge, skills, attitudes, behavior, or condition. OR The measurable effect on someone as a result of what they did in your program. A number & percentage of clients will accomplish Y for/in Z amount of time For example: 75% (10) of participants will report a 40% increase in how often they read to their children

How did you set the target? Do you have a benchmark? Long-Term Effects How will achieving your outcome(s) benefit the community? The client? This may extend beyond the terms of the grant. These components are what the consolidated application funders ask from all programs seeking funding, but most places that request outcomes expect these parts.
