1 A: An oblique view of the right foot shows multiple large corticated periarticular erosions about the first metatarso-phalangeal joint. The erosion at the medial base of the proximal phalanx of the great toe has an “overhanging edge (arrow).” Notice the relative preservation of the 1st MTP joint space, despite the numerous large periarticular erosions. There is periarticular soft tissue swelling. The findings are classic for gout. Multiple similar erosions are also present about the 2nd through 5th tarso-metatarsal joints. This distribution is also frequently seen in gout. B: A lateral view of the left great toe shows large corticated periarticular erosions with overhanging edges about the first metatarso-phalangeal and interphalangeal joints. Marked soft tissue swelling with central density is present on the dorsal aspect of the first interphalangeal joint. The findings are classic of tophaceous gout. Source: Chapter 10.
