5 Historical Literature on Medical Semiotics
German and Russian doctors in the 1800’s wrote about medical Semiotics, such as Feodorovich and Hippius (1892)’s work Semiotics and Diagnosis of Childhood Diseases (Semiotik und Diagnostik der Kinderkrankheiten). Or Hufeland (1823) in the Journal of Obstetric Practice who wrote On the value and importance of semiotics (Ueber den Werth und die Bedeutung der Semiotik), similarly Becker (1832) addressed the role of semiotics in relation to cardiology in his On the physiology and semiotics of cardiac activity (Zur Physiologie und Semiotik der Herzthätigkeit). This literature naturally, culminates in the works of Thure von Uexküll, as the one of the originators of a modern movement in medical semiotics and psychosomatic medicine (von Uexküll, 1986, Von Uexküll, 1982, von Uexküll, 1979). 9 The Humanising Turn in Medicine
In Western Medicine, the dominant model that is taught in medical schools is the biomedical model based on clinical trials Rationalist approaches in medicine have been contested Empiricists since the early 1900s by medical reformers such as Sir William Osler, “The practice of medicine is an art based on science” (Silverman et al., 2003). This movement reappeared in 1960/70 American and also its infancy in the University of Glasgow in the 1980s In 1993 General Medical Council (GMC) started to recommend the incorporation of medical humanities modules in its guidance for trainee doctors Tomorrows Doctors (GMC, 1993). 10 The Humanising Turn in Medicine
Which function as an additional course for medical students, that come with additional credits but are by no means a core part of the medical curriculum. As of 2013 humanities modules are available to medical students in 30 of the UK’s 32 medical schools (REF, 2014). 11 Thure von Uexküll Thure Baron von Üxküll (1908– 2004) was a German scholar of psychosomatic medicine and biosemiotics. He developed the approach of his father, Jakob von Uexküll, in the study of living systems and applied it in medicine. 16 Discussion Thure von Uexküll’s work on Clinical Education yet to be explored in any semiotic context. With the recent launch of Edusemiotics it seemed time to explore this undercurrent of Uexküllian literature. In the table there are titles concerning medical regulation - an introduction to Semioethics (See (Petrilli, 2014, Petrilli, 2010, Petrilli and Ponzio, 2009) in a medical curriculum? Self et al. (1989) who found statistically significant changes in reasoning in medical students who studies ethics INSERT PICTURES OF CHOICE INSERT PICTURES OF CHOICE

Structure of the medical field can’t be modified just by attempting to change it structures Some changes in demographics are noticeable however as a result of ‘widening participation activities’ since the 1970’s there has been great increases in the numbers of female, ethnic and minority students as well older student (BMA, 2009). However if one looks at the economic profile of medical students, there has been no significant change in the economic background of the students admitted since 1970. Some such as Bloom (1988) have suggested that curricula reform in clinical education is merely a distraction: “medical education’s manifest humanistic mission is little more than a screen for the research mission which is the major concern of the institutions social structure.” The research mission here referring to the economic benefits generated from research grants, which Bloom argues is the real driving force behind curricula development. There may be one chance of due to the nature of labour migration in medical schools that could successfully contribute to the introduction of Semiotics into medical education: “In the UK…the medical schools with the highest research profiles have tended to retain largely traditional medical curricula, while the newest medical schools, which generally have lower research income and prestige, typically claim to have ‘innovative’ curricula which integrate science with clinical practice. This may represent attempts on the part of the new schools to symbolically differentiate themselves from the dominant players in the field, rather than to attempt to compete on the same terms” (Brosnan, 2009, p63) Hence Cardiff – Traditional to modern INSERT PICTURES OF CHOICE INSERT PICTURES OF CHOICE

20 Conclusions Within Thure von Uexküll’s numerous works there is an stand of literature devoted to clinical education. All of which is in German, and so remains to be translated and introduced to the English speaking world. Translating the titles of these works, suggests that Semioethics might offer a route to introduce semiotics into medical school curriculum. The translations of the works themselves is in its infancy and has not revealed any insights so far. It remains to be seen when body of work is finally translated, if such insight exist into how to introduce medical semiotics back into medical education.
