2 1- A sick or premature infant can not be fed initially or at all until several weeks or months old
2 – An infant in weaned prematurely because of illness in the infant or mother 3 – An infant who was not previously breastfed develops an allergy or food intolerance 4 – A mother who has lactated weeks, months, Or years earlier wants to nurse an adopted infant 5 – A mother who is nursing a biologic child wants to nurse an adopted child ( without benefit of pregnancy)

3 1 – Relactation The ability of a woman who has stopped breast feeding to start again. No matter how long ago This is possible even if a mother has not breastfed for many months or even years. It is more common for mothers of babies who require special care . Occur earlier when lactation has only recently stopped

4 2 – Induced Lactation The ability of a woman who has never been pregnant to produce breast milk and breast feed. Seems strange , but it is common in parts of the world as a normal physiological assurance . Both needs: To have a strong desire to feed her babyتمایل قوی Well motivation انگیزه خوب Considerable support for her effortحمایت Both : Almost the same methods of stimulating milk production

5 Produced breast milk method
1 – Mechanical stimulation Suckling of baby (the best stimulation) Breast and nipple massage Pump or hand expression ( Lact – Aid ) Borrowed baby – from a friend or relative Partner to stimulate the breasts 2 – Hormonal Stimulation Prolactin by nipples stimulation (in a non-lacting breast stimulates growth of the alveoli) Oxytocin only works once milk is secreted Psychological state of the woman is so important and needs to have be well supported from the beginning

6 Managing Relactation

7 If the baby is willing to suck :
Breastfeed as often as possible (every hours at least times in 24 hours) Sleep with the baby Encourage the baby to suck as long as possible Use both breasts at least minutes on each side Ensure good attachment and positioning (leaning and lying may be helpful) Check on the baby’s intake ( Urine output by weighing the nappies, each time may have a slightly different weight) No use of bottle If supplements are necessary , use a cup or gastric tube or breastfeeding supplementer

9 Milk may begin to appear between 1 – 6 weeks
An adequate milk supply may take several weeks to achieve Women may additionally require pump (electric or hand) and drug treatment

10 If the baby is unwilling to suck :
Ensure the baby is not ill Increase the skin to skin contact Carry the baby in a sling next to breast( kangaroo care) Offer the breast regularly Use a breastfeeding supplementer to encourage the baby to breastfeed Stimulate the breast and nipple by hand or dual pump (electric pump is better that hand pump) No use of bottle If the baby has been using a dummy or bottle, rub it over the nipple and areola ( baby may respond to the smell) Mother and baby bath together ( may help the baby to rediscover the breast)

11 Treatment

12 Specially women who have never been pregnant or who have not
breastfed for a very long time may find the following drugs are useful: 1 – Metoclopromide which increase prolactin 10 mg. 3 times a day for days 2 – Chlopromazine mg. 3 times a day for 7 days 3 – Hormones Oestrogens, progesterone In women who have never been pregnant, act similar to pregnancy by stimulating and growth the alveoli A few days after the hormones stop, breastmilk supply 4 – Other drugs Domperidone Sulpiride Oxytocin nasal spray ( RCCT) Only by consult the doctor

13 There is no significance difference in the milk of
mother who relactate or induces lactation compare with the mature breastmilk of other mothers. Of course , the women who have never been pregnant, do not produce colostrums but at 5 days their milk has similar amount of total protein, alpha-lactalbumin and IgA

14 Helpful suggestions: - Psychological factors (Breastfeeding is a confidence game) - Rest - Diet and fluid - Skin – to – skin contact - Massage her back - Massage the nipples just before breastfeeding - Sit down for 5 – 10 minutes before breastfeeding - Have a warm drink - Listen favorite music while breastfeeding (Mozart) Each drop of the expressed milh would be valuable !
