1 DANOS JOB SAFETY AND ENVIRONMENTAL ANALYSIS TRAINING
Hello and welcome to the Danos Job Safety and Environmental Analysis Training. The purpose of this training is to create a better understanding of the specific items that comprise our JSEA as well as the expectations that we should adhere to in our day-to-day safety planning.

2 JSEA Overview The Danos job safety and environmental analysis serves to address the task-based hazards and implement controls in order to perform a job safely This process is to incorporate all members of the work team and will be accompanied with a safety meeting and site walk-through prior to the start of work (read slide)

3 JSEA REVIEW FORM overview
This is an overview of our Review form, which is the first page of our JSEA. As you can see, there are multiple sections to input information and the major sections that encompass this page will be outlined in detail on the following slides.

4 JSEA Review Form This is the location of where the work is set to commence. (ex. Chevron WR-718) The specific location within the site (ex. West side cellar deck) If the customer has a permit-to-work system, this document is associated with a permit number (ex. 1212) The type of permit that is associated with the customer permit-to-work (ex. Hotwork, coldwork) This is an overview of the task (ex. Installing filter skid using pneumatic tugger and associated rigging Identify the “worst-case scenario” of injury/illness that a worker can sustain while doing this job (ex. Crush injury due to failed rigging, broken finger, etc.) (click through and read explanations)

5 JSEA Review Form Examples: working at heights, hot work procedures, crane operation safe work practices If the job requires deviation of customer or Danos written procedures (ex. Cannot install top rail on scaffold). This requires implementing Management of Change process (read the first boxed item then click through for explanation to that item, then click forward) All prescription medications are to be reported to the Danos and/or customer supervisor. Employees taking any unapproved medications will not be allowed to perform the job Are all members of the work team physically capable of performing their assigned job tasks?

6 JSEA Review Form Task Leader—the person leading the work. In many cases, this may or may not be the Danos supervisor (ex. Foreman, painter/blaster, fitter, etc. (click once to see the boxed item, then read the explanation for task leader) (click forward and read explanation of UWA but read below as well) The ultimate work authority needs to be identified and signed onto the JSEA prior to the start of work. According to SEMS, in the event that someone calls “stop work”, the UWA has to review the reason for stopping the job as well as validate that it is safe to proceed with work. It is important to remember that we can stop the job for any reason, but if the reason presented an imminent danger to employees, equipment, facility, or the environment then it is the UWA who has the authority to allow work to proceed. An example of this is stopping an employee who is walking through a work site without the proper gloves. This instance would not require UWA verification to get back to working as it did not present an imminent danger to the employee. If a group of employees were walking through an area where crane operations were being conducted directly overhead and were not paying attention, this is an example of shutting the job down and seeking UWA approval as crane operations are considered by BSEE as critical in nature. If in doubt, stop the job and seek UWA approval. It is better to be safe than sorry. (click forward and read task leader explanation) As defined by SEMS, the “Ultimate Work Authority” is the authority assigned to an individual or position to make final decisions relating to activities and operations on the facility. The supervisor may also be the task leader, but may also be the customer or Danos supervisor on-site

7 JSEA Review Form This portion outlines the expectations of all members that are tasked with performing work or will be performing work in a nearby location (simultaneous operations) (click through until boxes pop up and read explanations, then click to next slide) The Work Site Verification portion outlines the expectations of the task leader and addresses the critical aspects of our Job Safety and Environmental Analysis program

8 JSEA Review Form In this section, include all the relevant personal protective equipment that will be associated with any phase of the job Any PPE not specified on the form should be included in the “other” section (ex. HRC 4 Arc Flash Suit) (read slide)

9 JSEA Pre-Job Assessment Form
This form is to be taken to the worksite and used to facilitate safe work planning. This section is not all inclusive! This form serves only to assist, and if there are any additional hazards that are identified, please take the time to implement the proper controls on the subsequent portion of the JSEA. It is expected that the required PPE for the activity and working conditions will be used at all times. INSTRUCTIONS: Use this form to assist in identifying the relevant hazards that are present throughout the job and incorporate the applicable hazard controls into the HAZARD MANAGEMENT section of the JSEA” This is the second page of our JSEA (click through and read explanations)

10 PERIODIC HAZARD REVIEW FORM
The Periodic Hazard Review Form is to be used during the workday as the job progresses. The “SHARP” Process of hazard recognition is used to identify any new hazards that may be present during the various tasks of the job. If you are an experienced employee you may not have seen this before in many of the Job Safety and Environmental Analyses you conducted in the past. This section is associated with the third page of our JSEA. (read through the entire slide and click through)

11 PERIODIC HAZARD REVIEW FORM
If after answering the questions, there are any additional hazards present, please note which steps (ex. 1,2 and 4) were added to or changed using “wet ink” These are the questions that will assist in the recognition of hazards as the job progresses. Please include all members of the work team in the answering of these questions After these steps have been completed, please note the time and task leader or designated person who is leading the work at that time. These are the intervals at which the work team will take a moment to identify additional hazards. (Click through and read)

12 HAZARD MANAGEMENT FORM
The same description as stated on the first page review form The potential hazards that are identified within each task is included here. Be specific in annotating these hazards (ex. “Slips/trips/fall” is unacceptable… Ask yourself, what equipment or condition can cause me to slip/trip/fall?) This portion includes the basic job steps that are necessary to complete the job. An important factor to remember is that this portion needs to be site-specific as well as task-oriented The hazard controls should specify the particular methods or equipment that is necessary to eliminate or mitigate these hazards. The person or persons assigned to each step of the job are included here. Each member that has a specified role needs to sign or initial their name onto each step This is the 4th page of our JSEA (click through and read all) There are 3 identical pages of our Hazard Management Forms, and if you need more room to annotated the required information, feel free to print as many pages as necessary.

13 AFTER ACTION REVIEW FORM
This portion outlines any potential learning or improvement opportunities in terms of identifying and mitigating hazards. The intent is to discuss the JSEA with the work team and determine any ways to improve on the next job. This form is required to be completed by all members of the work team after the job is done. The purpose of this After Action Review form is to address that there were no injuries, illnesses, equipment damage, or environmental issues directly related to the completion of their specific job tasks. This is the last page of our JSEA. This form is another that you may not been expected complete in your previous JSEAs or prior to working at Danos. (click through and read all) It is acceptable that you perform this portion of the JSEA at the end of the workday if need be. It is not uncommon that we work under multiple JSEAs during the day, and a good practice is to discuss and “close out” the JSEAs after finishing the job or while on a break later in the day. The JSEA is not considered completed until this form and process is done.

14 ADDITIONAL INFORMATION
The Danos job safety and environmental analysis process and forms are somewhat different than other companies throughout our industry in terms of requirements and expectations The “SHARP” process of hazard recognition was implemented to ensure that we are addressing hazards that can occur as the work progresses. The intent is to keep the JSEA at the worksite and use it as a guide to identify and mitigate these hazards. The AFTER ACTION REVIEW form is associated with ensuring that we report any incidents or injuries that have happened while we are conducting work. In the interest of maintaining the high Danos standard and expectation of reporting incidents, this form is required to be completed at the end of the daily job.
