Scheduled Invoice Total Balance Due = Wage Based Balance + HW Balance + Risk Balance The Scheduled Invoice is the unit’s overall invoice. It incorporates all premiums payable per program or plan, including Risk, Health & Welfare, and Wage based benefits. The body of the invoice lists: Each particular premium due, such as Clergy Supplemental Health, Dental, Medical, etc. The unit’s previous balance per plan The most recent payment applied Any applicable adjustments Followed by the current billing period’s premium charge and the amounts due, again per plan. The Total Balance Due reflects the sum of the Balance Due column. Occasionally, MCC receives unapplied payments. If this is the case, that information will appear in the “Payments received but not applied” area. To ensure this does not happen, you’ll want to make sure to forward the remittance voucher/vouchers attached to the invoice when paying.

Remittance Consolidated Remittance Partial Remittance
Use when paying in full Partial Remittance Use when not paying in full When paying the balance in full use the Consolidated Remittance. Individual coupons are used only when you pay a portion of the amount due, and want to apply it ‘per program’. If you see a balance in the “Payments received but not applied” line, just call MCC’s Finance Department to do so.

Health and Welfare Details Report (HW)
Dental, Medical, Clergy Auto and Supplemental Health Details The HW, or Health and Welfare, Details Report is the itemization of all medical, dental, clergy supplemental health and auto insurance premiums for the coming month. The report lists each participant individually, itemizes the benefits he or she is enrolled in, then provides a corresponding Employee Total. At the end of this report, the number of participants is listed as well as the Total Premium charged.

Wage Based Details Report
Lay Retirement and Unemployment Details The Wage Based Details Report is an itemization of the Lay Retirement and Unemployment premiums. Again, each participant is listed individually, followed by the wages used to calculate his or her unemployment premium, and the respective premium charged. These charges, remember, run two months behind as they are based on the wages submitted for each employee. At the end of this report is a concise summary which includes the number of participants listed and the Total Premium without Clergy.
