1 Settings of care Your choice about where you want to be cared for.
Leicester City Clinical Commissioning Group West Leicestershire Clinical Commissioning Group East Leicestershire and Rutland Clinical Commissioning Group Settings of care Your choice about where you want to be cared for. NHS Leicester, Leicestershire and Rutland (CCG) are the people who plan and buy the health care services in your area. Health care services are things like your doctor’s surgery and hospitals. Some people will need someone to help them to read the information so that they understand it.

It has some information and some questions for you to answer. 3 OR The NHS wants to make sure you have a choice of where you are cared for. The NHS wants to make sure they look at all your needs to give you the best care. The NHS wants to make sure that care is fair for all patients. What is NHS Continuing Healthcare? NHS Continuing Healthcare is when the NHS pay for the care that you need over and above what you get: At the hospital, At your doctors surgery (including your medication) By your NHS community team. People who need a lot of help because of their health may get NHS Continuing Healthcare. You will get NHS Continuing Healthcare if you need a lot of help in the day and at night because of your health. It may be because you have: Had an accident. Are disabled. Have an illness. We will use the word NHS Continuing Healthcare when we are talking about this. 2

4 3 The NHS will decide if you can get NHS Continuing Healthcare.
They will then sort out all the things that you need to help you with your day to keep you well. Then they will make a plan for all your healthcare needs. They will talk to you about your healthcare needs and put it in a care plan. What is settings of care? Settings of care is a small part of NHS Continuing Healthcare. People who need help and care don’t need to stay in the hospital unless they need medical care. People who need help and care need support to do things like: Going places Getting ready Taking their medicines We are looking at how we can help and care for people in places where they like to stay. People are much happier when they stay in a place they like. 3

5 4 You have a choice of where you want to be cared for.
These places are called settings of care. These places may be in: Your own home – where you live with your family or on your own. A nursing home – where you live with other people who need care and nurses take care of you. A residential care home – where you live with other people who need care and carers take care of you. We will use the word settings of care when we are talking about these places. What does the NHS want to do? Now you have read about settings of care we want to know what you think. We have questions for you to answer. It will help us to think about what is best for the people that get NHS Continuing Healthcare. All your answers will be kept private and we will not be able to tell who you are. Some people may need someone to help them answer the questions. 4

The NHS has to make sure that the money is used in the right way. The NHS has to make sure we do not waste money. The NHS needs to make sure we use the staff, equipment and other things for a patients care in the best way. These things are called ‘resources’.
