3 Benefit Classifications
Your average hours per week are measured at 3 times: Your first 6 weeks of working for FHC March – August (for October 1 eligibility) September – February (for April 1 eligibility) If you average hours = Full-time Eligibility If you average 20.0 – = Part-time Eligibility If you average or less = Not Eligible Hours are measured on a per week average Family Home Care – FHCcares.com –

6 Medical Provider is Group Health HMO (Health Maintenance Organization)
All locations excluding Boise = 2 plan options (HMO and PPO) Boise = 1 plan option (PPO) First 4 outpatient visits and $500 diagnostic covered before deductible required HMO (Health Maintenance Organization) Basically means you MUST stay in network to receive coverage unless it’s a true emergency Lower bi-weekly cost, $4,000 individual deductible, $6,000 out of pocket max PPO (Preferred Provider Organization) Can go outside of network and receive coverage at reduced rate Higher bi-weekly cost, $2,500 individual deductible, $4,000 out of pocket max Family Home Care – FHCcares.com –

7 Waivers Waivers If you do not sign up, you will need to sign a waiver. (Some benefits come by just working for Family Home Care. No hourly requirement!) If you need to take the paperwork home to discuss with someone… You need to have the paperwork back within hours In the meantime, you will be asked to sign a waiver When you bring the paperwork back, updated, the waiver will be disposed of, changed if necessary, and you will be signed up for benefits.
