Resident Benefits Orientation

2 Who is Eligible to be Covered?
For medical, dental, life/AD&D, flexible spending accounts – all Residents who are scheduled to work at least hours per week; plus spouse, dependent child and qualified domestic partner. For long term disability – all Residents who are scheduled to work at least 75 hours within a pay period.

3 When Does the Coverage Begin?
All coverages begin on July 1st. Within next 5 days, go to to enroll (page 4 of Benefits Guide), use logon instructions on the website landing page. If you are not enrolled or waive by July 1st you will be automatically enrolled in the HMO plans with single coverage and no changes can be made. If you don’t want the coverage, you must go online to WAIVE your coverage.

4 Medical & Rx Plans * Medical insurance through Empire BCBS
* Two types of Plans: EPO & PPO * (EPO) Exclusive Provider Organization provides in-network only coverage, no need to name a Primary Care Physician (PCP). * (PPO) Preferred Provider Organization provides both in-network and out-of-network coverage. * See page 7 of Benefits Guide for details about the EPO and PPO. * Rx is provided through Express Scripts.

5 Dental Plans * Dental insurance through Delta Dental
* Three Plans: DHMO and two PPO’s * (DHMO) DeltaCare provides in-network only coverage, must name a Primary Care Dentist (PCD). Not available in all states. * (PPO) Preferred Provider Organization provides both in-network and out-of-network coverage, lower co-insurance and annual max. * (PPO Premier) Preferred Provider Organization provides both in-network and out-of-network coverage, higher co-insurance and annual max. * See page 9 of Benefits Guide for details

6 Your Contributions Your per-pay-period contributions are listed on
page 10 of the Benefits Guide. Medical/ Rx contributions are based on the plan you choose, your salary tier, and level of coverage (single, family, etc.) Dental contributions are based on your plan and level of coverage. All contributions are deducted on a pre-tax basis.

7 Vision Care Vision care is provided through EyeMed.
EyeMed is not insurance but a discount plan. Use an in-network provider and receive discounts on lenses, frames and examinations for everyone in your family. See the EyeMed flyer for details and a discount card.

Life/AD&D Insurance Lutheran provides, at no cost to you, $100,000 in life insurance and $100,000 in accidental death & dismemberment (AD&D). You can purchase supplemental life insurance for yourself, your spouse and any dependent children. Guaranteed Issue is the amount you are guaranteed to be covered for with no medical questions. This is only available now as a new hire. See page 11 of the Benefits Guide for details.

Disability Insurance Lutheran provides New York State short-term disability insurance, which pays 50% of your base weekly salary to a maximum of $170/wk. Lutheran provides, at no cost to you, long-term disability insurance, which pays 60% of your base salary up to $1,500/month. You can purchase supplemental short- and long-term disability, and critical illness coverage through Farmington Benefits. See the Voluntary Benefits flyer Farmington’s Customer Service number.

10 Other Valuable Programs
If you should become a victim of identity theft, Lutheran provides access to Identity Theft Remediation Services. While you are traveling at least 100 miles from your home, Lutheran provides access to Travel Assistance services. See the ID Theft and On Call flyers for details

11 Flexible Spending Accounts
Two types of Flexible Spending Accounts Healthcare Flexible Spending & Dependent Daycare Flexible Spending Healthcare account used for unreimbursed medical, dental, Rx and vision costs. Pre-tax annual maximum = $2,500. Dependent daycare account used for daycare, afterschool, summer camp costs for children under 13. Pre-tax annual maximum = $5,000.

12 Flexible Spending Accounts
Plan Year is January 1st – December 31st. Must re-enroll every year. IRS regulations do not allow changes during the Plan Year. If you do not use it, you will lose it. Use a debit card or file a claim to be reimbursed.

13 Two types of Commuter Accounts
Mass Transit & Parking Mass Transit account is used for bus, subway, train, ferry, light rail expenses. Pre-tax monthly maximum = $130. Parking account is used for parking expenses in a garage, in a lot or at a meter. Pre-tax monthly maximum = $250.

14 Commuter Accounts Plan Year is January 1st – December 31st.
Your balance rolls over each year. Able to make changes every month. Use a debit card or file a claim to be reimbursed.

15 403(b) Retirement Plan Eligible to make contributions immediately.
VALIC administers the plan, contact to enroll. 2014 Pre-tax annual maximum = $17,500.
