You must fill in the fields with asterisk (*) You cannot complete your application unless you fill in these fields You must fill in all necessary fields carefully, completely, accurately, properly according to the instructions in order that your applications for residence permit will be evaluated If you choose "mobile phone" as a contact preference and complete the pre-registration process, a verification code will be sent to your phone. In order to continue your residence permit application process, you must enter the verification code in "verification code" field

28 REQUIRED DOCUMENTS Passport
Four Passport sized (portraid) photos (photo must be taken within the last 6-month (frontal, clear face, in a way that the foreigner can be recognized easily) with a white background and as biometric) Photocopy of passport pages (the page with your photo, the page where validity and expiry dates are written, and the page of the last entry) In cases where passport is not written in Latin letters, certified translation by a sworn translator or official authorities shall also be added to the application The payment receipt and the copy of the receipt for residence permit card. The payment amount is 58,5 TL If you participate Erasmus Internship Programme, you need to pay the visa fee which is 353,10 TL to the Hisar Tax Office (if you have Erasmus Internship visa,you do not need to pay the visa fee)

31 TAX ID FOR RESIDENCE PERMIT CARD FEE
In order to pay the residence permit card fee, you need to have Tax ID number. In this regard, you need to visit the tax office with your passport. Please see the address of the Tax Office below Address of Beyoğlu Vergi Dairesi (Tax Office): Meşrutiyet Caddesi, No: 125, Şişhane/ Beyoğlu

32 PAYMENT FOR THE RESIDENCE PERMIT
You need to pay 58,5TL You can pay the Residence Permit card fee in two tax offices that are listed below HİSAR VERGİ DAİRESİ (SİRKECİ) Address :  Hobyar Mh. Yalıköşkü Cd. No:16-20 Sirkeci /Fatih GELİR İDARESİ BAŞKANLIĞI ( VATAN CADDESİ )  Address: Akşemsettin Mahallesi  Adnan Menderes Bulvarı    (Vatan Cad.) No:56 Fatih Make sure to get a receipt of the payment

33 WHERE TO SUBMIT THE DOCUMENTS
You need to submit the all required document with the application form at İstanbul Migration Office on your appointment date. Address :  Hirka-i Serif Mahallesi Vatan Caddesi No:64 Fatih/ISTANBUL

34 LEAVING TURKEY BEFORE RECEIVING THE RESIDENCE PERMIT
After the submission please be sure that you have got your «Müracaat Belgesi» It means the receipt of application In case you decide to leave Turkey before you receive the residence permit, please make sure you have your Müracaat Belgesi+ residence permit card fee payment receipt with you at the airport that shows you applied for your residence permit According to the law, you are NOT allowed to leave the country more than 15 days without residence permit

35 HEALTH INSURANCE Starting Fall 2014 Turkish authorities ask for proof of minimal health insurance coverage to issue the residence permits Option 1 Bring your health Insurance Policy from your home country (Only valid for Erasmus Exchange Students) Option 2 Social security agreement between Turkey and your home country Option 3 Private Health Insurance (Allianz)

36 OPTION 1 HOME COUNTRY HEALTH INSURANCE ( ONLY VALID FOR ERASMUS EXCHANGE STUDENTS)
If you are bringing your own health insurance from your home country, it should cover the minimal coverage The coverage table has to be shown in your insurance policy explicitly. Your insurance policy must be in compliance with the minimal requirements of the mandate for obtaining a residence permit in Turkey It has to be translated in Turkish and approved by the Notary in Turkey. Please be aware that this regulation is only valid for Erasmus Exchange Students

37 OPTION 2- HEALTH INSURANCE
Turkey has health agreements with some countries listed below. Students who are from one of these countries are exempt from paying to General Turkish Health Insurance fee Please bring the original “Medical Assistance Entitlement Certificate" and submit it to the Social Security Directorate in Istanbul with a copy of your passport and the student certificate.
