8 Client Numbers Increase with AmeriCorps and Care & Referral Centers Open
Vision for Assisting Unsheltered Homeless in Lompoc and Santa Maria Americorps Lompoc: Overseeing 2 Members (40 hours outreach per week) Santa Maria: Overseeing 2 Members (40 hours outreach per week) Coordinated Outreach Coordinating local agencies to partner and house individuals with 18 months of housing retention supportive service Lompoc: Tuesdays 2pm; Santa Maria: Wednesdays 11am

Once a week regular events with incentives to bring unsheltered homeless to us to receive local services Lompoc: Trinity Church of the Nazarene Saturdays 1pm Santa Maria: Salvation Army Lunch Program Wednesdays 12pm

10 With Increased Coordinated Outreach and Care & Referral in Place
We have 23 being assisted in Lompoc – outreach and retention We have 16 being assisted in Santa Maria – outreach and retention We have doubled our number of clients in North County on Vertical Change I currently lead both outreach teams – 20 official clients – 19 others being assisted in some way We have reached our housing goal in Lompoc for 2016!

4 individuals housed in December/January 1 more scheduled for move in mid January Total 5 in housing in 2016; 9 in housing retention All housed have remained housed – 100% Retention Initiated 2 applications with Landlord Liaison Partnership Current partners – Lompoc Police Department, Behavioral Wellness, Public Health, Americorps, Trinity Church, Lompoc Advocacy & Action, Bridgehouse, Coast Valley Treatment Center, Transitions Mental Health, HACSB, Landlord Liaison Partnership

12 Who Comes to Care & Referral in Santa Maria?
Partners – Public Health, Behavioral Wellness, Good Samaritan Shelter, Americorps, City of Santa Maria, Transitions Mental Health, C3H Unsheltered homeless individuals Families doubled and tripled up Average attendance - 85 Vulnerability surveys, housing applications, client agreements, other service provider applications and/or agreements Data entered on Vertical Change weekly Working to increase partners in 2017

13 Santa Maria Story Met Mother & Daughter at Care & Referral at Salvation Army AmeriCorps member took them on as clients – family has some income Assisted family with filling out an application for Landlord Liaison Program LLP wrote a letter of recommendation to the Landlord C3H partnered with Crestwood to pay landlord application fees Family to move into their own apartment in January!!! C3H will provide housing retention plan

Working with the City of Buellton and People Self Help Housing with plan for residents of Avenue of Flags motels Assisting People Helping People with outreach to community to fill gaps with their clients – Kickers Soccer & Local Project Assisting as Needed with homeless in Santa Maria Individual – last week finished VI for individual 68 years of age

Quarterly Coordination Meetings – highlighting local partners, discussing immediate needs, reports from COT and Care & Referral Weekly Coordinated Outreach and Care & Referral Data entry and sharing on Vertical Change Outreach for more Flexible Local Dollar funds: Lompoc: Trinity Church. Santa Maria: Crestwood Christian Fellowship.

17 Santa Maria Outreach - streets, vehicles Common Ground maps community, recruits volunteers, and creates teams for outreach from 6-9pm Santa Maria Police Department & 6 local volunteers do outreach in vehicles from 9-11pm Care & Referral Center Centro Christiano Vida Abundante 6-9pm Shelters, Warming Centers – fill out form

20 In Conclusion: The Process Works – 4 Steps to Solving Homelessness
Know everyone by name – expanding with 80 hours per week of outreach through AmeriCorps has helped us get to know more people Know what they want/what they need – using Vertical Change county wide helps all partners know the needs and wants of the unsheltered homeless community Line up/expand resources – having partners work together decreases gaps in services. Increasing partners in the community (LLP/DWW/FLD) expands resources that meet needs and wants of the unsheltered homeless community HOUSING WITH RETENTION – creating teams with intention of 18 months of supportive services is proven effective nationally and providing true in North County!
