2 Your Well Being: Healthy U Wellness Campaign “Healthy U” Wellness Campaign This campaign will provide opportunities for employees to gauge current health status and their healthcare needs to develop a customized program to ensure a healthy you. The campaign will hands-on tools to assist in developing a road map to good health and resources to meet your specific health needs. 2

3 Your Well Being: “Healthy U” Campaign Three Easy Steps to starting the path to a “Healthy U” 1.“Opt In” for 2016 as part of online open enrollment process on myUT portal 2.Complete Biometric Screenings conducted by UT Rocket Wellness Services by 12/31/2015 3.Complete online Health Risk Assessment (HRA) by 12/31/2015 3

4 Your Well Being: Health Assessment & Risk Evaluation Biometric Health Screenings must complete by 12/31/15 UT Rocket Wellness Staff, will contact those employees who opt in to “Healthy U” Campaign to schedule their personal screening. One of the first steps to a healthy lifestyle is learning your health risk factors and your “numbers” including cholesterol, blood sugar blood pressure, height and weight. 4

6 Your Well Being: Health Assessment & Risk Evaluation Online Health Risk Assessment - conducted by chosen health care provider – online The Health Risk Assessment (HRA) questionnaire is a tool that gives participants an evaluation of their current health and quality of life. The HRA promotes health awareness by reviewing participants’ personal lifestyle practices and revealing health issues that could be impacted by personal choices. 6

8 Your Well Being: “Healthy U” Campaign “Healthy U” Personal Wellness Plan Effective January 1, 2016 all enrolled employees in the “Healthy U” Campaign will have the opportunity to meet with a UT Rocket Wellness Coach to customize a specific health plan based on their health assessment. This is voluntary and strictly confidential. Additionally, programming and health challenges will be developed to meet the health needs of our employees. Behavior Modification and Lifestyle Management classes will be offered to employees that are assigned as part of the Personal Wellness Plan Health Challenges Examples of health challenges that will be offered include: “Maintain Don’t Gain”, “Fall into Fitness”, and “Smart Choices for a Healthy Weight”. 8

10 Your Well Being: “ Healthy U Campaign” Sample Programs 10 ProgramExamples Supporting Care and Self Care Symptom Management Services will be used to coach employees on preventative and lifestyle management strategies. This includes pediatric and adult health. Health Challenges Access to REC Center Healthy Lifestyle Educational Series: Healthy Eating Stress Management Time Management Healthy Relationships Specialist Disease Management Lifestyle management and behavior modification programming will be offered. These disease states include: asthma, diabetes, heart disease, stroke risk, musculoskeletal, C.O.P.D. Employees are enrolled in disease specific classes: (sample classes) Diabetes Care and Management (12 Week Program) Heart Disease and Weight Management (10 week Program) Exercise is Medicine (10 Week Program) Case Management Intensive one on one telephonic/onsite care and education with a team to be identified Case management will be integrated with medication therapy management (MTM) care through Pharmacy Employees will be enrolled in personalized case management programs to ensure compliance and adherence.
