2 History  Established in 2001, maintained with the Creating Healthy Communities grant administered by the Athens City-County Health Department with additional funding provided by the Communities Preventing Chronic Disease grant and the Appalachian Diabetes Translation and Control Project grant

4 Geographic Focus: Athens County, OH

5 Athens County Snapshot  Ranked 72 of 88 of health behaviors (tobacco, obesity, food environment index, etc.)  28% of Athens County adults are physically inactive  30% of Athens County adults use tobacco products  Food environment index is 5.2 (measuring access to healthy foods and percentage of population considered food insecure on a 0-10 scale)

8 Athens County Plan to Prevent and Reduce Chronic Disease  Adopted from Ohio’s Plan to Prevent and Reduce Chronic Disease  Promotes policy, system, and environmental change related to health in five sectors:  Community  Government  Worksites  Healthcare  Schools & Universities

9 Athens County’s Plan to Prevent and Reduce Chronic Disease: Successes  Partnership with Rural Action to bring Farm to School to local preschools and daycare centers  Implementation of new mile marker signs on the bike path and new bike lanes to support active transportation  Complete Streets workshop on October 19  Healthy checkout lanes at local grocery stores  Nelsonville Farmer’s Market and Athens City Schools Food Pantry

12 The Appalachian Health Summit Event goals: Increase local enrollment in lifestyle change programs (DPP & CHIP) by 5% through provider-based referrals based on information received at the Appalachian Health Summit

14 Diabetes Prevention Toolkit  Includes lifestyle change program referral form, Healthy Connections Clinic flyer, information on the Diabetes Prevention Program, information on the Complete Health Improvement Program, Check It Change It Control It resources, community health workers information, diabetes prevention information from the Rural Health Information Hub and the American Diabetes Association, local resource guides, etc.

15 Athens County Public Library Partnership  Planned installation of iHealth blood pressure monitors at each of the seven branches in the Athens County Public Library system  Train librarians on orienting community members to health- related resources at the library  Kick-off events will be planned for each library with cooking demonstrations, physical activity, and unveiling of iHealth monitors
