3 WHAT’S NEW FOR 2017  Kaiser Permanente HMO will replace the Cigna EPO plan beginning January 1, 2017  There is a Medicare Complement Component under Kaiser similar to the UHC Medicare Complement plan  Coverage added for virtual visits (telemedicine) at no cost (Kaiser) or only a $10 copay (UHC)  Retirees enrolled in the UHC Medicare Complement plan are not eligible for the UHC virtual visit benefit 3

4 COST SHARING  The cost share for all employee groups remain the same  Retirees pay 20% for all plans 4

5 RATE CHANGES  Decreases  The rate for the United Healthcare Point of Service (POS) plan will decrease by 2% ($2.40 less per month for single coverage)  Staying the same  Caremark/Silver Script Prescription  Vision Plan  Increases  Dental plan will increase by 2% ($0.15 cents per month for single)  UHC EPO will increase by 1.8%. ($1.60 cents per month for single)  UHC Medicare Complement (Retirees over the age of 65) will increase by 6.4% ($3.40 more per month for single coverage) 5

11 United Concordia Dental PPO Plan  Members have the freedom to choose any licensed dentist for service  You are NOT required to select a dentist at the time of enrollment.  You may pay more out of pocket if you choose a out-of-network dentist.  To find a participating provider visit United Concordia Dental’s website at www.unitedconcordia.com to access the provider directory or call customer service at 1-866-851-7564www.unitedconcordia.com  It is the member’s responsibility to submit Out-Of-Network claims 11

13 Annual Benefits Open Enrollment Simple Steps to Follow  Refer to the open enrollment letter from the Executive Director, Patti Barney  Review open enrollment documents and forms which are available on the M-NCPPC website at www.mncppc.orgwww.mncppc.org  From the home page, under Quick Links, click on the 2017 Open Enrollment link  Retirees who don’t have access to the internet may request a hard copy of the enrollment packet or a specific form from the Health & Benefits Office  Complete and submit benefits enrollment/change form and any required documents to Health & Benefits by November 4 at 5 p.m. ET. 13
