4 Site Noise Committee Committee Membership  Facilities  Industrial Hygiene  Health Services  Manufacturing Engineering  Safety  Others as Needed Roles & Responsibilities  Ensure completion of the site sound survey  Ensure the site has a functioning noise abatement program that focuses on the list of the top five (5) noise control priorities and/or areas and to document those activities.  Participates in any incident investigations associated with a confirmed STS+ diagnosis  Complete the annual Sound Status Report (SSR)  Annually reviews compliance to the Hearing Conservation & Noise Control Self- Assessment and Regulatory Compliance tool.  Periodically evaluate the effectiveness of the Site Hearing Conservation and Noise Control Program.

5 The Sound Survey Process SL 2.0 Sound Level Specification since its inception in 1980s A systematic approach to estimating the exposure of employees to noise Traditionally performed by an outside contractor due to the time intensive nature of the testing and creation of the report

6 Collection of Noise Samples  A representative, task-based sampling approach is used as opposed to dosimetry*  Job Function Profiles created toestimate exposure of similarly exposed people  Use the Larson Davis Spark NoiseDosimeter to measure sound levels
