3 Improvement Strategies By March 31, 2020, seniors who require community support can remain at home as long as possible. If needs progress to requiring long term care, it is a priority to ensure Seniors have the highest quality of care possible.

4 By March 31, 2017, the number of clients with a Method of Assigning Priority Levels (MAPLe) score of three to five living in the community supported by home care will increase by 2%. *Ministry alignment In 2015/16, increase the number of LTC resident units where all residents receive purposeful rounding by 10% each month. *Ministry alignment By March 31, 2016, the use of antipsychotics without a diagnosis of psychosis will be reduced from current levels in long term care. *Ministry alignment By March 31, 2016, the use of daily physical restraints will be reduced from current levels in long term care. *Ministry alignment By March 31, 2016, reduce the number of falls experienced by home care clients. By March 31, 2016, 100% of long-term care residents will have meaningful life care plans. By March 31, 2016, reduce sick and overtime hours in long-term care facilities by 10%. Improvement Goals

15 Corrective Actions What did we learn in 2015/2016 that will impact the improvement work for 2016/2017? – RPIWs Purposeful Rounding – will meet targets Enhancing Home Support Exercise Program Improving Client Engagement in Falls Prevention – Audits will measure if the activities noted in Meaningful Life Care Plans are part of the Resident’s life.

17 Opportunities RPIWs in Home Care: – have ignited interest in improvement work – time to focus on a priority improvement purposeful working together specific tools - everybody loves the 5S! By working together horizontally and vertically, we can find synergies to make improvements. Replication work will be key with time frames for implementation.

18 Opportunities The engagement of clients has been an incentive to the Home Care Staff and Leaders: HSEP, Falls Prevention program Gratitude to staff who remain dedicated to improving the quality of our service.
