Why Health Policy?

The US Health Care System is complex Patient advocacy and optimum service delivery require “system thinking” Physicians are often asked for an opinion regarding policy; ought to speak from informed position – Not that it has stopped anyone yet

Role of physicians with health policy: Physicians effect change Physicians are affected by change

And then, there’s the ACA Arch Intern Med. 2012;172(20):1603-1605. doi:10.1001/archinternmed.2012.3758

AAMC graduation questionnaire Between 2003-2007, a total of 55,294 senior medical students responded to the questions about their education in the practice of medicine. – Fewer than 40% of students reported appropriate training, especially in medical economics. Highly variable educational time is devoted to health systems among medical schools. – Students in those schools with higher intensity training in health care systems reported increased satisfaction. Acad Med. 2009 Sep;84(9):1301-6. doi: 10.1097/ACM.0b013e3181b17e3e

Why Jeopardy? Based on experience with COM I and II students, it was noted that there was a lack knowledge regarding health policy. – Although there were several lectures regarding ACA, health policy was not a high priority in the curriculum – Not tested on NBME The goal was to find a way to have fun and get information across effectively.

Content and process Literature review on appropriate content for COM discussion – Affordable Care Act and HITECH – Quality Improvement – PCMH – Value based care – Evidence based medicine Used VALUE –Framework to organize information into topics (J Gen Intern Med 27(9):1210–4) Used Jeopardy as format due to global familiarity and competitive nature of medical students

How to create a Jeopardy Game Commercial products are available Google “Making a Jeopardy game using PowerPoint” – Have to already have PowerPoint – 100 questions took about 3 hours to put together Answer key or no?

Student feedback: Learning styles

Students: What would you change? Most comments mentioned either providing more information before or after the game Suggestions for improvement:  Speed of the game, couldn’t always process the information in the timeframe  Shorten some of the Jeopardy “answers”

Future Directions: Call for a standardized medical school curriculum in health policy. DomainsComponents Systems and Principles US health care system, financing and payment Models of care management and control Health insurance Health care safety net Health information technology Physician Workforce Quality and Safety Quality indicators, measures and outcomes Quality improvement Patient safety Value and Equity Medical economics Medical decision making Comparative effectiveness Health disparities Politics and Law History and consequences of major healthcare legislation Adverse events, medical errors and malpractice Patel MS, DAVIS MM, LYPSONML.
