2 The Quentin N. Burdick Memorial Health Care Facility is located in Rolette County, on the Turtle Mountain Indian Reservation, Belcourt North Dakota. QUENTIN N. BURDICK MEMORIAL HEALTH CARE FACILITY

3 The Quentin N. Burdick Memorial Health Care Facility is a 27 bed Hospital providing 24 Hr Acute Care (Medical/Surgical/Obstetrics/Pediatrics) and Emergency Room services. The QNBMHCF Active User Population—14,321. Acute Care consists of 12-Adult Medical Beds; 6-Adult Surgical Beds; 6- Obstetric Beds; 3-Pediatric Beds; and 6 Newborn Bassinettes. The Hospital provides Ambulatory Care, Emergency Room, Medical, Surgical, Pediatric, OG/GYN, General Surgery, Podiatry, Dental, Optometry, Behavioral Health, Dietary, Laboratory, Radiology, and Community Health. The QNBMHCF Radiology services includes Ultrasound, 24/Hr CAT Scan, and Mobile MRI services 3 x month. Renovations of Emergency Room—Summer 2013

8 QUENTIN N. BUDICK MEMORIAL HEALTH CARE FACILITY The QNBMHCF is the first birthing facility in North Dakota to be recognized as “Baby Friendly” through Baby Friendly USA. The Baby Friendly Hospital Initiative (BFHI) is a global Initiative of the World Health Organization (WHO) and the United Nations Children’s Fund (UNICEF). This initiative is designed to get hospitals to adhere to the Ten Steps to Successful Breastfeeding. Breastfeeding is the natural biological conclusion to pregnancy and an important mechanism for the continued normal development of the infant. With the correct information and the right support in place, most women who choose to breastfeed are able to successfully achieve their goal. Because the process of attaining Baby-Friendly designation compels facilities to examine, challenge and modify longstanding policies and procedures, the process also strengthens the organization by enhancing leadership skills, increasing staff competence, and improves patient satisfaction. Although the hospital is not, and should not be, the only place a mother receives

9 QUENTIN N. BUDICK MEMORIAL HEALTH CARE FACILITY QNBMHCF has initiated Improving Patient Care (IPC) as a new and innovative way of providing improved patient care. IPC will allow each patient to form a close relationship with a Primary Care Provider (PCP) as well as a designated Care Team. A Care Team consists of a group of dedicated and reliable healthcare professionals who utilize the highest standards of care to improve the health, wellness, and quality of life for the people who utilize QNBMHCF. Patients may choose a clinical care team and will be encouraged to see their designated care team. Each patient will know their care team members and each care team member will know their patient. Patients will have better access to care from their Primary Care Provider. Patients will benefit from Increased communication and consistency in care. Patients will have access to better tools/education for self-management or prevention of Chronic Disease.

BUDICK MEMORIAL HEALTH CARE FACILITY QNBMHCF Leadership and front line staff are involved in the Partnership for Patients (PfP) initiatives: IHS PfP initiative will span 18 months to improve patient safety and quality. PfP Initiatives/Goals: Decrease Hospital-Acquired Conditions (HACs) and Readmissions Keep Patients from getting injured or sicker. By end of 2013, preventable hospital-acquired conditions would be decreased by 40% compared to 2010. Help Patients heal without complications By end of 2013, preventable complications during a transition from one care setting to another would be decreased so that all hospital readmissions would be reduced by 20% compared to 2010.

13 QUENTIN N. BURDICK MEMORIAL HEALTH CARE FACILITY The Turtle Mountains offer a lot of outdoor activities that the whole family will enjoy. The Turtle Mountains present a long list of great destinations:  Bottineau County – is one of the original counties of the territory in 1872. It was named for Pierre Bottineau, a Metis pioneer, hunter and trapper who became a successful land speculator. The county is well known for its winter park, snowmobiling, and ice fishing.

14 QUENTIN N. BURDICK MEMORIAL HEALTH CARE FACILITY  Rolette County – is where you can find the International Peace Garden. The International Peace Garden is a park located on the international boarder between Canada and the United States. It includes an 18-foot (5.5m) floral clock display, fountains, a chime, and twin 120 foot concrete towers straddling the border with peace chapel at their base.  Dunseith— Founded in 1884 and named for Jeanette Dunseith Eaton, who was the mother of the town’s founder William Eaton. Dunseith is known for its proximity to the International Peach Garden. The name Dunseith is actually a Gaelic word that means “City of Peace”.

15 QUENTIN N. BURDICK MEMORIAL HEALTH CARE FACILITY  International Peace Garden—is the 2,339-acre Botanical Garden that was devoted to world peace. Thousands of tourists are attracted to the garden. It provides intriguing displays and a colorful tropical garden. The garden has become a symbol of friendship between North Dakota and Manitoba. It showcases reflecting pools and dazzling colorful floral displays of over 150,000 species of flowers splashed across the grounds of the Formal Garden’s terraced walkways. It is filled with abundant animals such as deer, moose, beaver, and others that form a great marriage with the existence of variety or wildflowers, trees, and shrubs.
