Examples of issues we support with; Lonely or isolated, Poor health, Poor housing /struggling to cope with housing /at risk of homelessness, Not linked in with other services, Need advice/support dealing with one or multiply issues, Those for whom old age has made life harder.

Investing in technology and practices we are sharing.

5 ALONE’s Business Plan 2016-2018 ALONE are investing €1.3 million to achieve core aims 1.To develop a National Infrastructure of 6/8 regional hubs suppling support coordination – currently supplying to AHB’S in 7 countries 2.Hubs linked locally to housing, health services & befriending services 3.To provide support services to other housing providers/community. 4.To develop with others agreed models to support older people age at home. 5.To champion Housing with Support and Care 6.To make the best use of technology to support older people to age at home

9 ALONE’s Befriending & Advocacy Befriending is a proven health intervention 1 in 3 older people live alone 1 in 10 suffer from chronic loneliness which shortens lives Provides companionship for isolated older people Volunteers can support older people with: Basic advocacy Keep people linked into the community Attend ALONE social events Volunteers are and must be trained, Garda vetted & supported Quality Award - ensures accountability/manages risk The community help to look after their own. If needs change, the volunteers and staff link them in with services in the community.

15 Support Coordination – The numbers ALONE’s Support Coordination Active Case = €250-€500 (including assessment, support plan & close out) Short Intervention = €25-€50 (information, advice and remote support) 60 day close out on 85% of all support objectives - (will stay as long as required) ALONE’s Support Coordination Case Load in 2015 was 1,061 International Figures 1.90 saved for every £1, spent* 23% improvement in peoples self reported wellbeing* 87% of practitioners say integration is working very well* 40% drop in acute admissions for long term conditions * (* References available on request)

Professional staff provide a Support Coordination service to residents (20% require at any time) Types of housing: schemes, pepper potting and one- off housing. 20 application for each house 98% occ rate 8 week,turnaround 15 % a year 960 maintenance call a year. 98% with time. All public/private /social housing to have % units for older people Long term this provides local moving options & access to support and housing with care When we are building schemes for older people we use Universal design, location and access to support. That we agree a model nationally on Housing with Care To allow people to ‘’trade down’ in to Housing with Care Using housing to keep pensions in the community
