6 PAC Charter 6 Purpose Statement The formation of the Project Advisory Committee (PAC) is State mandated and is being created for the purpose of promoting collaboration with peer partners and other community-based providers. The Committee has an advisory role. It advises the PPS Executive Committee on all elements of the DSRIP Project Plans and will be managed by the PMO. Goals and Aims Goal 1Ensure successful collaboration of PPS partners Aim 1Provide a confidential environment to allow PPS partners to evolve as healthcare associates Goal 2Receive quarterly updates from the PMO or Committee Chairs as to the progress of projects or operations Goal 3Provide a forum for PPS partner leaders to attend meetings to be informed of PPS progress Goal 4Act as an advisory body for the planning, implementation, or outcomes of projects, policies, or processes

7 PAC Charter Deliverables Output 1Review the Community Needs Assessment & create a work plan to identify a list of projects to be presented to the Transitional Steering Committee for project approval for submission to DOH Output 2Inform committees to promote productivity, confidentiality, and collaboration Output 3Suggest quality initiatives, evidence based protocols, and performance metrics for project metrics & milestones to workgroups for plan submission Output 6Partner with consultants and the Project Management Office Output 7Support public comment processes as needed to meet State requirements Output 8Review implementation progress reports from project operational points of contact Output 9Assist with institutional issues identified during implementation or ongoing maintenance Output 10Review the PMO published bi-annual PPS report to include accomplishments, lessons learned, metric and milestone progress. (To be presented to the Executive Committee) 7
