Paper vs. Presentation Paper 2-4 page Double Spaced Times New Roman Font 12 1” margins Title page with your name, date, class and purpose of case study Source/citation page with APA formatting. (See me if you need help with this.)

Objective What is your overall objective for the case study?  What information would you like to provide?

Anatomical Structures What are the main anatomical structures that were involved in the injury? (Be Specific)  Do not use terms such as Leg, Arm, Finger  Try and use pictures, make it interesting (Presentation)

Case Background Patient Info (No Names) Sport Info Position played at the time of injury Injury Mechanism (How it happened) BE SPECIFIC.

Evaluation Initial Evaluation  Who performed the evaluation?  What did you find or see?  What did you do ? Follow Up Evaluation  Who performed the evaluation?  What did you find or see?  What did you do ? Physician Evaluation  What did he or she find or see?  What did they do?  What type (if any) diagnostic test were ordered?  What were the results?

Differential Diagnosis What are all of the possible injuries?

Final Diagnosis After ruling out all of the other possible injuries what is the final diagnosis ? How did you or the physician come to this diagnosis?

Treatment What are the treatment options? ( If there is more than one) Which was chosen for this athlete?

Follow Up Care How often will the athlete be evaluated for progress?

Outcome What is the overall outcome of the case? How did the athlete do? Are they still suffering from the injury? Are they 100%?

Uniqueness What makes this case interesting or unique to you?

Sources What sources did you use to get some of the background information? APA format:
