Computerized Provider Order Entry Medication Orders Configuring RPMS-EHR for Meaningful Use Resource Patient Management System

Presenter: Mike Allen, MIS, RPh IHS OIT EHR Pharmacy Consultant Carla Stearle, PharmD, BCPS IHS OIT EHR Pharmacy Consultant CPOE Medications

Core MU Objective “Use Computerized Provider Order Entry (CPOE) for medication orders directly entered by any licensed healthcare professional authorized to enter orders into the medical record per state, local, and professional guidelines.” - 42 CFR Part 495.6,(d)(1)(i) CPOE Medications

Applicability Eligible Professionals (EP) Eligible Hospitals Critical Access Hospitals (CAH) CPOE Medications

Training Objectives Use CPOE for medication orders. Optimize drug file in conjunction with Pharmacy Informaticist. Use medication quick orders appropriately for medication ordering. Discuss differences between MON report and MU report on CPOE. CPOE Medications

Core Performance Measure Type of Measure: Rate The number of unique patients in the denominator who have at least one medication order entered using CPOE >30% The number of unique patients seen by the provider during the EHR reporting period who have at least one medication in their medication list CPOE Medications

Threshold More than 30% of all unique patients with at least one medication in their medication list seen by the provider during the EHR reporting period have at least one medication order entered using CPOE. CPOE Medications

RPMS Reporting Logic Denominator Inclusions: COUNT: each patient: WHERE: one or more medications are present as structured data on the patient’s medication list AND WHERE: the patient had one or more face-to-face visits with the provider CPOE Medications

RPMS Reporting Logic Numerator Inclusions: COUNT: each patient in the Denominator: WHERE: one or more medications has a “Date Issue” during the EHR reporting period AND WHERE: the “Nature of Order” for the counted medication is not = “written” AND WHERE: the prescription was entered by a licensed healthcare professional holding the ORES or ORELSE key AND WHERE: for EPs, the order was entered, signed, and released to the service CPOE Medications

RPMS Reporting Logic Measure Exclusion: EPs who write (enter) fewer than 100 prescriptions during the EHR reporting period are excluded from this measure. No exclusion for eligible hospitals and CAHs. CPOE Medications

Configure RPMS CPOE Medications During our how-to session we will: 1.Demonstrate how pharmacist sets up a drug and builds a quick order to facilitate CPOE. 2.Utilize medication quick orders appropriately for medication ordering in EHR. 3.Demonstrate differences between MON report and MU report on CPOE.

Evidence of Functionality During our how-to session we will: Order a medication in EHR and check that: – The medication can be found. – Medication order details are correct. – Signing the order changes the medication status to pending on the Meds tab. CPOE Medications
