4 MU Objective and Measure Provide patients with an electronic copy of their discharge instructions at the time of discharge, upon request. More than 50% of all patients who are discharged from the hospital’s inpatient or emergency department and who request an electronic copy of their discharge instructions are provided it. 5 MU Measure Type of Measure: Rate The number of patients in the denominator who are provided an electronic copy of discharge instructions. >50% Number of patients discharged from an eligible hospital's or CAH's inpatient or emergency departments (POS 21 or 23) who request an electronic copy of their discharge instructions and procedures during the EHR reporting period. 6 RPMS Reporting Logic Denominator Inclusions: COUNT: each patient HAVING: requested an electronic copy of discharge instructions during the EHR reporting period HAVING: a hospitalization during the EHR reporting period (defined as Service Category of H) WHERE: the Discharge Type was Regular Discharge, Transferred, or Irregular Discharge OR HAVING: an emergency department visit during the EHR reporting period (defined as clinic code of Emergency Department-30 and Service Category of A) Electronic Copy of Discharge Instructions

7 RPMS Reporting Logic Numerator Inclusions: COUNT: each patient in the Denominator WHERE: the patient was given an electronic copy of discharge instructions at the time of discharge Electronic Copy of Discharge Instructions

8 RPMS Reporting Logic Measure Exclusion: An eligible hospital or CAH that receives no requests for electronic discharge instructions during the reporting periods is excluded from this requirement. Electronic Copy of Discharge Instructions

9 What HIM Needs to Know Understand the MU objective and the measure If patient is requesting an E-Copy of Discharge Instructions If provider or nurse provides patient with E-Copy Examine the use of the TIU Note Titles - Discharge Instructions; “E-Copy Discharge Instr Not Received” and “E- Copy Discharge Instr Received” Discharge Instructions templates available on the EHR/FTP site Generate TIU/SSD list of titles to verify that E copy was received or not received Document disclosure in the ROI application as necessary Electronic Copy of Discharge Instructions
