Registration Two ways to register for SRS 1. Online at – you may request access to any of our online systems, including SRS. 2. Via the Company’s SRS Company Administrator – SRS Administrators who have security access to SRS sub system(s), can register new users. They can also provide and manage security roles to new and existing users. 5

Register and Manage Users 7 Select Administration and “Register Company User”. Enter persons details and click “Register”. Add security roles. You can only provide Representative and Administrator roles for sub systems you are Company Administrator. You can only have a maximum of three Administrators per sub system. Use same process to “Manage Company Users”.

User Access Review The review is to undertake regular ‘housekeeping’ to ensure only current company users have security roles and access to SRS. Company administrators have the ability to revoke roles of previous users. An Alert is sent to all company administrators every three months to conduct the review. o Administrators are to complete the review within one week. o A reminder will be sent if review not done. 9

Notifications – Monthly Status Report The Monthly Status Report is found in the Notifications sub- system. The Report must be submitted within15 days of the end of the reporting month: –Mines Safety and Inspection Regulations 1995, r –Mines Safety and Inspection Levy Regulations 2010, r. 32 Information provided is used to: –determine performance data for compiling injury statistics –analyse performance trends –calculate mining levy 15

Levy changes 1 July 2014 r. 3 definition of worker has been changed Worker means an individual who for remuneration carries out work at a mine in the course of mining operations (whether under a contract of employment, a contract for services or other arrangements). r. 31(2) has been replaced The principal employer at a mine must keep records that enable the total number of hours worked at the mine by workers in each month be verified. 20

Notifications – Injury A mining injury notification should be submitted under S.76 and R. 3.41: –Injuries to be reported where the accident results in a disabling injury [lost time(L), alternative/restrictive/light duties (A)] –Notification should be completed at the end of accident month and submitted within two weeks of following month –Where the injury occurs at end of month and the start date of the work status is in following month, the notification is to be completed at the end of that month and submitted within 2 weeks Information provided is used to: –determine performance data for compiling injury statistics –analyse accident trends and provide information and guidance 23

Notifications – Notifiable Incident The Notifiable Incident Report Form acts as immediate notification for reporting accidents and incidents. s.76, 78 and 79 r Details provide vital information on the type of incident. Information provided is used to: –determine performance data for compiling injury statistics –assist in investigations –analyse accident trends and provide information and guidance 25

Notifications – Type of Incident Each incident type is defined by Regulation. Type 14 – “potentially serious occurrence” is where there is no injury or harm and is not applicable to any other Type listed.. 27

Related Communications and tasks Related Communications (Rel Comms) facilitates communication between industry and the Department. Enables easy tracking of queries and information. Is a one-stop shop to target specific individuals to provide relevant information and guidance. Provides accountability in the creation of a record of a communication issued and received. Links to the communications can be accessed from within: –Individual’s system –SRS Home Page – via Alerts or Tasks –‘Related Communication’ within the Notification itself 30

Approvals – Project Management Plan (PMP) A Principal Employer or manager must not commence mining operations at a mine until approval of State Mining Engineer has been procured in writing (s. 42(3)). Approval request is completed online and submitted to District Inspector where project is located. PMPs are required for: –New Operation –Major Expansion / Change to Existing Operation PMP provides information on the actual project (e.g. name of company and site, geology, facilities, project overview). 35
