Integrated primary healthcare nursing What does ‘integration’ mean? No single, accepted definition of integrated healthcare Most include references to care co-ordination, seamlessness, person centeredness, and whole-of- system working together New models of care development What does ‘models of care’ mean? Northland’s guidance on models of care MoH - consistent themes are about; Person/ whanau centred reflective of community needs, resources and culture Team based approach What are our objectives? Increase patient experience Fit for purpose Meets community needs over next 20 years Coordination of nursing services

Define principles a moral rule or a strong belief that influences your actions a belief that is accepted as a reason for acting or thinking in a particular way

World Café questions to develop the principles What are the principles that we will base our design of an integrated primary care nursing? e.g. Nursing workforce will work to the top of the scope as is possible Why did we choose this principle? e.g. in 20 years time there will be significant shortage of nursing workforce Think about person and patient related, workforce related, access related, health system related and other.
