Hypo,Hyperthyroidism and Hashimoto Thyroiditis Pathology

Hypo,Hyperthyroidism and Hashimoto Thyroiditis Understand the concept of an endocrine organ Define hypo and hyperthyroidism. Define Hashimoto thyroiditis. Recognize the causes and the pathogenetic mechanisms involved in hypo, hyperthyroidism and in Hashimoto thyroiditis. Describe the pathological findings in cases of hypo and hyperthyroidism and Hashimoto thyroiditis.

Hypo,Hyperthyroidism and Hashimoto Thyroiditis Learning objectives: The student should: Know the ways in which thyroid disorders present. Know the major causes of hypo and hyperthyroidism. Know the causes of a goiter. Know the causes of a solitary nodule in the thyroid. Understand the classification and behaviour of thyroid carcinoma.

Endocrine System Autocrine Paracrine Endocrine In endocrine signaling, the secreted molecules (hormones) act on target cells that are distant from their site of synthesis. An endocrine hormone is frequently carried by the blood from its site of release to its target.

Thyroid Gland The thyroid gland consists of two bulky lateral lobes connected by a relatively thin isthmus, usually located below and anterior to the larynx. The thyroid gland is one of the most responsive organs in the body and contains the largest store of hormones of any endocrine gland.

Thyroid Gland

Hypothyroidism Hypothyroidism is caused by any structural or functional derangement that interferes with the production of adequate levels of thyroid hormone: 0.3%-- subclinical hypothyroidism: > 4%. Age Sex Hypothalamic-pituitary-thyroid axis. Primary and secondary categories

Hypothyroidism PRIMARY Developmental (thyroid dysgenesis: PAX8, FOXE1, TSH receptor mutations) Postablative, Iodine deficiency Surgery, radioiodine therapy, or external irradiation Autoimmune hypothyroidism Hashimoto thyroiditis* Congenital biosynthetic defect (dyshormonogenetic goiter)*

Hypothyroidism SECONDARY ( CENTRAL ) Pituitary failure Hypothalamic failure (rare)

Hypothyroidism Cretinism: severe mental retardation, short stature, coarse facial features, a protruding tongue, and umbilical hernia. Myxedema: slowing of physical and mental activity: mental sluggishness-overweight. Glycosaminoglycans and hyaluronic acid, in skin, subcutaneous tissue, visceral sites. Non-pitting edema, a broadening and coarsening of facial features, enlargement of the tongue, deepening of the voice.

Thyrotoxicosis Graves : 1835 : "violent and long continued palpitations in females" associated with enlargement of the thyroid gland. Graves disease Hyperthyroidism owing to hyperfunctional, diffuse enlargement Infiltrative ophthalmopathy (exophthalmos) Localized, infiltrative dermopathy (pretibialmyxedema)

Thyrotoxicosis Graves disease is an autoimmune disorder : (autoantibodies to the TSH receptor are central to disease pathogenesis): LATS IgG antibody : anti-TSH receptor Coexistence of stimulating and inhibiting immunoglobulins in the serum of the same patient, a finding that could explain why some patients with Graves disease spontaneously develop episodes of hypothyroidism.

Thyrotoxicosis

Thyroiditis Thyroiditis: inflammation of the thyroid gland Acute illness with severe thyroid pain (e.g., infectious thyroiditis, subacutegranulomatousthyroiditis) Disorders : little inflammation, thyroid dysfunction (subacute lymphocytic thyroiditis and fibrous [Reidel] thyroiditis).

Hashimoto’s Thyroiditis Hashimoto thyroiditis is the most common cause of hypothyroidism in areas of the world where iodine levels are sufficient. The name Hashimoto thyroiditis 1912 report by Hashimoto describing patients with goiter and intense lymphocytic infiltration of the thyroid (struma lymphomatosa). Hashimoto thyroiditis and Graves disease Female predominance of 10 : 1 to 20 : 1.

Hashimoto’s Thyroiditis Hashimoto thyroiditis (or chronic lymphocytic thyroiditis) is the most common cause of hypothyroidism in areas of the world where iodine levels are sufficient. Gradual thyroid failure by autoimmune destruction of the thyroid gland

Hashimoto Thyroiditis Hashimoto thyroiditis is an autoimmune disease in which the immune system reacts against a variety of thyroid antigens. The feature of Hashimoto thyroiditis is progressive depletion of thyroid epithelial cells (thyrocytes), replaced by mononuclear cell infiltration and fibrosis.

Hashimoto Thyroiditis

Conclusion An increase or a decrease in the thyroid hormones secretion lead to different clinical and pathological changes. Causes, pathogenetic mechanisms and the histological findings of hyper and hypothyroidism.
