Six Kentucky Counties

The Grant Can Support Evidence-based interventions Use of existing County Cooperative Extension Service offices and community partners Interventions aimed at improving physical activity, nutrition and obesity reduction.

Short-Term Healthy Eating and Physical Activity Outcomes Increased knowledge of healthy behaviors associated with eating and physical activity. Increased number or capacity of existing community coalitions Increased number of community-wide of healthy practices

The Extension Agent Educator, Facilitator, and Catalyst Identifies issue needing to be addressed Discusses with council or committee to develop ways of addressing issue Plans evidence-based programs that are fun and educational to help teach participants how to eliminate the identified issue Implements activities to help participants practice what they have learned Helps evaluate project

Menu of Options Coalition choices include activities to support each of these three strategies: Provide community-wide programs to increase knowledge to increase healthy eating and physical activity behaviors. Increase access to and promote healthier food retail. Increase opportunities for physical activity through joint-use agreements, Safe Routes to School, walk/bike programs, walkability and other assessments.
