Up-to-date medical notes from the GP means the group are much better informed to make the right decisions about care for these vulnerable and challenging clients, and it is a stable and quick and reliable platform to use." Dr S. Mackay-Thomas (Frailty MDT Lead) “Has certainly helped me as it means I have an accurate list of patients medication and have access to previous results which gives me a more detailed idea about the patients’ diabetes care up to that point.” Dr S.
