I.e.; dermatology, cardiology, speech therapy 4)Does your dependent require special housing or equipment? If you answered YES to any of these questions, you have a family member that may meet criteria for EFMP enrollment.

EFMP-M Out-processing Single members and Mil to Mil with no dependents - Proceed to the front so we can take your information and release you from vMPF Accompanied Stateside Assignment with no EFMP member -Fill out an AF form If you answered No to every question, please proceed to the front. -If you answered Yes to any question, the following information applies to you: Accompanied Stateside Assignment with EFMP members -The four forms below will be required only from the member who is enrolled in the EFMP Required Forms: - AF Form 1466 Demographic sheet (filled out by everyone) 1 per family - AF form 1466D (Dental) filled out by dentist for anyone over the age of 2 - DD form 2792 (Medical) must be filled by provider/psychiatrist; 1 per member - DD form (Education) must be filled out by DoDDS for children over 3 Exceptional Family Member Program
