LANCE ARMSTRONG FOUNDATION Founded in 1997 by Lance Armstrong Unite people to fight cancer Take aim at the gap between what is known and what is done Our mission: To inspire and empower people affected by cancer Unity is strength. Knowledge is power. Attitude is everything. LIVESTRONG

CDC Funded Projects

GOALS To deliver cancer education and awareness activities to minority and medically underserved (MMU) cancer survivors and their families. To disseminate current, comprehensive information on cancer survivorship to MMU cancer survivors and providers that serve them.

INFORMATION DEVELOPMENT Through the CDC Cooperative Agreement, the LAF has developed the following resources to reach MMU cancer survivors: –Living After Cancer Treatment brochure series –Cancer Survivorship Training Curriculum for Promotoras –Addendums to the National Action Plan for Cancer Survivorship –LIVESTRONG Podcast Series for Young Adults with Cancer –Spanish-language Outreach Project

BROCHURE SERIES Serves as an introduction to cancer survivorship for the following populations: –Adolescent/Young Adult –African American –American Indian/Alaska Native –Appalachian –Asian American –Arab American –Chinese American –Hispanic/Latino (English & Spanish) –Multicultural –Pacific Islander –Vietnamese American Written at 6 th grade reading level Available at livestrong.org/brochures

PROMOTORAS CURRICULUM The Cancer Survivorship Curriculum provides the knowledge, skills and motivation to improve the quality of life of Hispanic/Latino cancer survivors. The primary audience for the curriculum is Promotoras in the state of Texas. The secondary audience includes Hispanic/Latino cancer survivors who access services provided by Promotoras in their community. The curriculum consists of four sessions that are dedicated to providing the tools and strategies to promote cancer survivors to keep track of their emotional, physical and day-to-day concerns.

ADDENDUMS to NATIONAL ACTION PLAN The LAF has created addendums to the National Action Plan for Cancer Survivorship for African Americans and American Indian/Alaska Native communities. These addendums prioritize the cancer survivorship needs of these two communities to ultimately lead to improved quality of life and survival rates for survivors. State agencies, organizations & individuals can use these priorities to increase awareness of the needs of cancer survivors.

LIVESTRONG PODCAST SERIES 12 video podcasts to increase Adolescent/Young Adult (AYA) survivors’ awareness of the concerns survivors face and the resources available to meet those concerns. Features AYA cancer survivors sharing there stories and will include role models as hosts for each podcast. AYA survivors will be able to download the podcasts free from the websites they access regularly. Available June 2008

SPANISH OUTREACH PROJECT The LAF is raising awareness of the physical, emotional and day-to-day concerns of cancer survivors among Spanish-speaking populations. –Creating –Producing 8 60-second radio spots that will air on Spanish radio stations across the country –Producing 5 one-hour call-in shows that will feature cancer survivorship experts. Shows will air in LA, Chicago, San Antonio, NYC and Miami Website & radio spots launch June 1st. Call-in shows begin airing on June 15 th.

EVALUATION EFFORTS The LAF evaluates each project with cancer survivors and healthcare professionals through an online survey. The brochures also include a pre-paid postage evaluation card that recipients can complete and return to the LAF. The LAF will track hits on livestrong.org/espanol and calls to LIVESTRONG SurvivorCare from Spanish- speaking cancer survivors.

MAJOR SUCCESSES Among cancer survivors who have received a copy of a Living After Cancer Treatment brochure, over 95% felt the brochure had helped them to identify their concerns about cancer. Of healthcare professionals who received copies of the Living After Cancer Treatment brochures, 100% would recommend that cancer survivors use the brochure to address their cancer-related needs. Over 572,000 brochures have been distributed. 85% of Promotoras trained with curriculum had an increased knowledge in cancer survivorship and the topics addressed by the program.

MAJOR SUCCESSES The LAF is committed to ensuring that each of these projects are sustained. We will support the distribution of the brochures and continue promotion of all online materials to cancer survivors and healthcare professionals.

Thank You! Claire Neal Director of Education & Program Development
