1 ADVANCING THE PRACTICE OF CASE MANAGEMENT
The practice of case management is a refined model of care coordination established more than a century ago (Joo, 2018). Case Management has been adopted into the American healthcare system but very little research exists that effectively demonstrates the strengths of a case management delivery model. The Agency for Healthcare Research and Quality identifies that 1% of high-risk patients account for 22% of all healthcare expenditures (Joo, 2018). The episodic and fragmented structure of the American healthcare system is responsible for these costs as well as poor quality care. Case management is an effective, efficient, and cost-effective strategy to deliver patient- and family-centered care. Case management is tied to care coordination in the literature, but consistency in definition and scope are lacking. CMSA was the first organization to define case management and establish Standards of Practice, but these are not universally adopted. For case management, and CMSA to be recognized as THE strategy for improved care coordination and patient outcomes, CMSA must develop a theoretical framework to guide research that will demonstrate these improvements and clarify to the healthcare industry who a case manager is, and the role the case manager plays in the care of patients. The Case Management Society of America Foundation envisions a better future for the case management industry through a collaborative, inclusive community of healthcare professionals working together to build industry-wide solutions to case management issues. Our Mission Our mission is to become the leading philanthropic organization for case managers, supporting education, research and professional development. If you’re not familiar with the CMSA Foundation, the Foundation supports CMSA Members by collaboratively to create a community of healthcare professionals to support case management education, research and professional development. . We have been around for a few years now and but excited to have recently gone through a rebrand and refocus on how we can support case managers and CMSA Members even better!

3 Goals Through an actionable plan initiated in 2011, the Foundation is working to achieve the following goals: Raising the visibility of case management and enhancing public awareness and esteem for the profession. Developing funding opportunities to support projects and programs that advance the practice of case management. Providing educational resources related to health care policy and practice issues impacting case management. Supporting the development of continuing education on critical case management issues. Promoting the application of new technology to support the practice of case management. read

4 Case Management Recognition
The Foundation supports and manages formal recognition of case management programs that demonstrate practice improvement or research. Current Award Opportunities: Case Management Practice Improvement Award Case Management Research Award Recipients are recognized at the CMSA Annual Conference, and in the CMSA Today magazine with an article highlighting their program and outcomes.

5 CMSA Foundation Scholarship and Grant Opportunities
A primary Foundation endeavor is to support the development of educational resources and raise awareness regarding health care policy and how it affects the role of the professional case manager. CMSAF is supporting research efforts through our grant and scholarship opportunities

6 Margaret Leonard Public Policy Grant
A new grant funding opportunity supporting public policy efforts. Government plays a major role in population health. Laws, regulations and policy influence how professional case managers practice within their role. Funding for this grant is made possible by Margaret Leonard, CMSA Past President and CMSA Foundation Past-President, who is dedicated to not only supporting the CMSA Foundation, but the promotion of public policy awareness. This grant supports the public policy efforts of an individual striving to build a program that effects case management. This grant is made possible by Margaret Leonard, and her unwaivering support and dedication, plus her love for public policy.

7 Margaret Leonard Public Policy Grant
The grant will be awarded to an individual case manager who has a passion for case management, advocacy and helping to change healthcare policy by effectively communicating with legislators, regulators at the local, state, or national level. The intent of this grant program is to assist an individual in creating a method to effectively promote and raise awareness regarding the Case Management Model Act, Standards of Practice for Case Managers, Transitions of Care, Care Coordination and other public policy topics supported by CMSA. Application will open no later than October 1. Application deadline for grants awarded in 2020 will be due by October See website for more details on the grant requirements and qualifications.

8 Integrated Case Management Training Program Scholarship
A scholarship to attend CMSA’s Integrated Case Management Training Program. This scholarship is offered to a case manager committed to improving health outcomes of the most at-risk populations and is willing to link and integrate care coordination across all domains of health. Need support in completing the Integrated Case Management training? Applications for the 2020 training be due in January Training consists of an online, self-study component, and then a 2-day face-to-face session in June 2020 during the CMSA Annual Conference.
