What is Hospice?  Compassionate Approach to the end of life.  Life expectancy is measured in months, rather than years.  Patient  Family Members/Love ones  Experienced clinical team  Dignity  Full Life  Personal control

Time to Choose Hospice When: Any time during life-limiting illness Curative treatments are no longer effective Life expectancy of six months or less Challenges: Patient/Families Accepting diagnosis Lack of knowledge Doctors Accepting diagnosis Loosing contact with patient Trouble conveying information of life expectancy

HOSPICE OFFERS HOPE : HOPE that a secure and familiar care setting can be enjoyed HOPE for personal dignity and freedom from unhelpful, and sometimes painful or uncomfortable procedures HOPE for freedom from the fears of isolation, abandonment, loneliness, loss of control and physical pain HOPE that the family will be nurtured and supported, even after the death of a patient.
