2 Who is Emmi Solutions? What is an “ Introduction to the Medical Home” Emmi program? Next Steps Agenda

3 Emmi is a healthcare communication platform -Engaging interactive content combined with an innovative technology platform -Focused on helping consumers take an active role in their health -Hospital, IPA, and Health Plan Customers -Over 1 million patients have been “prescribed” an Emmi program -Programs span chronic condition management, acute care, and patient safety WHO ARE WE?

4 Emmi Solutions is developing a Multimedia program to help practices communicate the medical home concept to patients in a consistent, understandable way - To be made available for free to physician practices and others - Help meet CMS pilot requirements for educating the patient on the medical home - Could also be used to educate providers and other stakeholders WHAT IS IT?

5 Emmi developed a similar program based on the Joint Commission “Speak Up” campaign This program was downloaded for free by approximately 400 hospitals
