Self Directed Support The process for providers

Self Directed Support “ You may of heard it being referred to as personalisation or personal budgets. There are different ways to describe it, but whatever name is given to it – its about giving people real power and control over their lives” In Control – Paul Ferguson 24 th Jan 2011

7 steps to Self Directed Support

Self Evaluation Questionnaire Asks questions about the kind of support a person needs Personal needs Work and learning Relationships Community Complex needs and risks Family Assessment of need is done by the individual with support from family, friends, social worker, provider (if needed)

Estimated Budget A points allocation system determines an estimated budget based on the self evaluation questionnaire Flexibility in how the budget can be spent – its not just about hours of support Resources are about more than just money – its about individual skills, strengths and talents, relationships, and community

Thinking about outcomes “The final product or end result The consequence of doing something Something that follows from an action” InputsProcessOutputOutcomes

Its about: Working together with people truly at the centre (coproduction) Identifying skills, talents and abilities and looking for opportunities to share these Understanding people in the context of their whole lives Outcome Based Support Planning

What’s in it? What you want to change and achieve The things that are important to you and give your live purpose and meaning The things that are important for you to be stay as healthy and safe as you can be The support you need to live the life of your choosing How you will use your budget Outcome Based Support Planning

Coproduction “Support that is designed and delivered in equal partnership between people and professionals’ Self Directed Support: A National Strategy for Scotland

Co-production “It is about involving people not only in the rowing and the steering of the boat, but also in actually building it.” - Mr Sandy Watson OBE DL, Chairman NHS Tayside

Getting the plan agreed The plan is sent to the local authority to be agreed. The resource allocation screening group will look to ensure that support needs are met and the plan keeps the person safe and well What if there is not enough in the budget to keep the person safe?

Organising money Direct Payment into the persons bank account or joint account with their guardian Money managed by a third party either – Individual Service Fund by a Providers – Trust Funds held by someone on my behalf Managed by the local authority A mixture of above

Organising support Support that’s right for you! How you want it! When you want it!

Live life and checking in to see how it’s going Regularly checking in to explore what’s working and what needs to change Monitoring and recording progress Ensuring money is being spent on agreed outcomes

Challenges and Opportunities? Working in groups, identify where you think some of the challenges and opportunities will be in the process. What first steps are you going to take back to your organisation?
