Must Pass Elements All six Must Pass Elements are required for recognition. Includes: 1A: Access During Office Hours 2D: Use Data for Population Management 3C: Manage Care 4A: Self-Care Process 5B: Referral Tracking and Follow-Up 6C: Implement Continuous Quality Improvement Total Must Pass Points = 29 (at 100% completion) © Qualidigm

Critical Factors These must be met for practices to receive more than minimal or, for some factors, any points. Includes: 1A-1*:Provide same-day appointments 1B-3: Provide timely clinical advice by telephone after hours 1G-2: Have regular team meetings / communication processes 3A-3: Report on a condition related to unhealthy behaviors / mental health / substance abuse 3D-1: Review and reconcile medications with patients / families 3E-2: Generate at least 75% of eligible prescriptions electronically 4A-3*:Develop self-management plans and goals 5A-1: Track lab tests 5A-2: Track imaging tests *Must-Pass Element © Qualidigm
